We are a boutique, relationship-first agency. Every client and family receives our full attention. We are looking for a compassionate, experienced Care Manager who takes personal ownership of client outcomes and caregiver performance.
The Role
This is a part-time, on-site and field-based position based in El Segundo, CA, with approximately 15 to 25 hours per week, scaling with agency growth. The Care Manager is the primary point of contact for clients and families on all matters related to care quality, and the direct supervisor of caregivers in the field. You will work closely with the Office Manager, who handles scheduling and administrative operations, and report directly to the agency owner.
This role is ideal for a seasoned home care professional who leads with empathy, communicates confidently with families, and holds caregivers to a high standard of care delivery.
What You Will Manage
Client Intake and Care Planning

What You Will Manage
Client Intake and Care Planning
- Conduct in-home assessments for all new clients to evaluate care needs, environment, safety, and family dynamics
- Develop individualized care plans that reflect each client's preferences, routines, medical considerations, and goals
- Match clients with caregivers based on skills, personality compatibility, language, and schedule
- Guide families through the onboarding process and set clear expectations for care delivery
Ongoing Client and Family Relations
- Serve as the primary care relationship contact for clients and their families
- Conduct regular check-in calls and scheduled home visits to assess client well-being and satisfaction
- Respond promptly to care concerns, changes in condition, or family feedback
- Identify early signs of client dissatisfaction and intervene before service is at risk
- Communicate care updates clearly and compassionately to family members
Caregiver Field Supervision and Performance
- Conduct field visits to observe caregiver performance and provide real-time coaching and feedback
- Ensure caregivers are following individualized care plans accurately and consistently
- Address performance issues directly and document concerns through the agency's systems
- Support caregiver confidence and skill development through mentorship and guidance
- Collaborate with the Office Manager on caregiver recognition and retention
Care Plan Management
- Update care plans promptly when a client's condition, preferences, or circumstances change
- Coordinate with the Office Manager to ensure scheduling reflects the most current care plan
- Maintain accurate, HIPAA-compliant client documentation within the agency's care management system
- Flag any changes in client condition that may require family notification or a higher level of care
Quality and Compliance
- Ensure all care delivered meets California Home Care Organization standards and agency policies
- Participate in quality reviews and contribute to continuous improvement of care delivery
- Support incident documentation and reporting as needed
- Stay current on best practices in private duty home care, dementia care, post-surgical recovery, and disability support
What We Are Looking For
Required
Required
- Minimum 3 years of experience in a home care, home health, or senior care setting in a supervisory, coordinator, or care management role
- Demonstrated ability to conduct client assessments and develop individualized care plans
- Strong interpersonal skills with experience communicating directly with clients, families, and caregivers in emotionally sensitive situations
- Ability to work independently in the field with reliable transportation
- Familiarity with California Home Care Organization (HCO) regulations and compliance standards
- Proficiency with care management or home care software for documentation and care plan management
- Valid California driver's license and reliable personal vehicle
Preferred
- CNA, HHA, LVN, or social work background with direct care experience
- Experience managing caregivers across multiple concurrent cases
- Prior experience with a concierge or private-pay home care agency
- Bilingual (English and Spanish) strongly preferred given the South Bay client and caregiver community
- Experience with post-surgical recovery care, dementia care, or disability support services
