JOB SUMMARY

This role provides comprehensive psychosocial assessment, counseling, care coordination, and resource navigation for patients and families receiving palliative care services. Collaborates with an interdisciplinary team to facilitate care planning, support transitions across the continuum of care, and advocate for patient and family needs.

SCHEDULE DETAILS

Location: This position is located 100%25 onsite.

Work Schedule: This position is scheduled for four 10-hour weekday shifts (8:00 a.m.–6:30 p.m.). No weekends or holidays. 
 

EXPERIENCE AND EDUCATION

Required

  • Education
    Master's Degree in Social Work from an institution certified by the Council of Social Work Education 
     
  • Licenses and Certifications
    Applicable state license required; Upon Hire and 
    (LCSW) LIC CLINCAL SOCIAL WORKER required or must obtain within 1 year of hire. 
     

Preferred

  • Experience
    3 years of experience providing direct clinical counseling services preferred. 
     
  • Licenses and Certifications
    certification in Advanced Hospice and Palliative Care preferred. Upon Hire 
     


JOB DUTIES

  • Serves as a liaison for transitional care planning between patient, caregivers and palliative care/clinical care teams. Assists clients to navigate healthcare systems; identifies and links clients with resources.
  • Attends administrative and clinical meetings relevant to operations of palliative care program; distributes relevant information to departmental staff. Participates in interdisciplinary team meetings, care planning and ethics consultations. Works with the multidisciplinary team to identify and develop disposition plans.
  • Responsible for independent psychosocial assessment and direct clinical counseling. Provides counseling and psychotherapy for individuals, couples and families. Provides grief, loss and bereavement support to patient and families. Provides psychosocial education to patients and family caregivers about coping strategies, hospice and palliative care philosophy and non-pharmacological symptom management strategies.
  • Utilizes crisis intervention and problem solving skills to facilitate and enhance patient/family's ability to meet acute and ongoing medical and psychosocial needs.
  • Advocates on behalf of the patient and family. Facilitates advance care planning.
  • Facilitates and/or supports existing psychoeducation support groups.
  • Provides inservices or education workshops to other service providers and/or organizations to promote the palliative care philosophy and program.
  • Maintains effective documents of social work activities.
  • Performs other duties as assigned.
