The Care Coordination Redesign Program works with people with HIV/ AIDS and seeks to connect them to medical services and stabilized housing. The Care Coordination Redesign Program (CCR) employs a navigation model and a chronic care model to advocate for, communicate with, and identify resources for the client as well as encourage and support clients to gain and maintain independence in their healthcare to ensure improved client health outcomes and improved quality of life.
- ESSENTIAL DUTIES & RESPONSIBILITIES:
- Responsible for direct, field-based services to enrolled consumers.
- Responsible for conducting Outreach at SRO field sites and community organizations to recruit and screen SRO residents who may be eligible for services.
- Responsible for conducting presentations and meetings with medical providers and community organizations to facilitate referrals.
- Conduct home visits to consumers to connect or re-engage them with medical providers.
- Identifies, research, and reports areas for potential consumers.
- Follows and updates the outreach referral log and weekly in-home visit schedules.
- Provide critical feedback to the CCR team based on observations in the field.
- Facilitate groups for CCR consumers as need.
- Participate in monthly team meetings, supervision, and training as required.
- Participate in Services for the Underserved Quality Assurance/ Quality Improvement activities
- Record all service provision on Field Logs (or “Case Finding”) for submission to the Data Entry Specialist on a daily basis.
- Generate monthly outreach reports
- Minimum 80% field in the community
- Perform other duties as directed.
Qualifications
REQUIRED EDUCATION AND EXPERIENCE
· Minimum Bachelor’s Degree.
· Experience working with people living with AIDS, homeless people or substance users.
· Effective written and oral communication skills.
PREFERRED QUALIFICATIONS & SKILLS
· Bi-lingual Spanish encouraged.
· Experience working within an outreach model or case management preferred.
· Basic computer skills (e.g. MS Office suite and Windows)
