Job Description:

Assess community behavioral health needs, provide input for facility strategic planning, business development, community outreach and maintain ongoing communication with key facility referral sources to promote information about Peace’s services; provide customer service with coordination of patient care; ability to discuss treatment/recommendations with referral sources.  Provide Level of Care Assessments as needed.

Essential Functions:

Under the direct supervision of the Director Behavioral Health Outreach, this position will provide the following:

  • Reviews referral source data and track trends from referral sources in territory
  • Initiates face to face contact with referral sources throughout the assigned region by conducting outreach visits to their agencies
  • Maintains positive referral source relationships in targeted regions
  • Responsible for processing referral source complaints working directly with the Director Behavioral Health Outreach and appropriate administrative personnel in developing strategies to resolve concerns and improve services
  • Participates in monthly department meetings. Completes other outreach duties as assigned
  • Abides by the Standards of Conduct in addition to all applicable state and federal regulatory requirements
  • Demonstrates a commitment to the presentation of a professional image in the workplace with regard to dress, personal appearance and hygiene

  • Exhibits willingness to learn/perform tasks outside the scope of his/her regular job duties and/or work additional hours/shifts as necessary to meet customer needs
  • Completes all assigned Annual Competencies and KNOW Modules by due date. Measure of Success: No deficiencies in completion of assigned KNOW modules
  • Maintains compliance with all company policies, procedures and standards of conduct
  • Complies with HIPAA privacy and security requirements to maintain confidentiality at all times
  • Performs other duties as assigned

Additional Job Description:

Education:

  • Master’s degree in a human services field (required)

Experience:

  • At least 2 years’ experience in a behavioral health setting working independently in marketing, business development or clinical setting (required)

Preferred Qualifications:   

  • Record of working with or building relationships with other community resources (preferred)
