The Role: Outreach Case Manager for our new Vista 29 property!

What You'll Do: You'll seek out homeless individuals and families at shelters, meal sites, day centers, streets, encampments, VA service sites, etc. You'll build rapport, screen for housing eligibility, provide direct assistance with accessing resources and supports, and refer clients to appropriate community services. Screen potentially eligible individuals and families for participation in the Housing First programs and other community services. Work with landlords, community services providers, and oversight entities to provide service, ensure responsiveness of our supportive services, and achieve high rates of housing stability and other consumer outcomes.

What You'll Bring to SupportWorks:

  • A Bachelor's degree preferred and at least one year of direct service experience with individuals with low-incomes, disabilities, and/or histories of homelessness.
  • Knowledge of mental illness, substance abuse disorders, community resources, and recovery concepts.
  • Skill in establishing supportive trusting relationships with persons experiencing homelessness and with professionals in the homeless services and Veterans services systems.
  • Ability to approach homeless individuals and build rapport through empathy, concern and understanding.
  • Willingness to work in all-weather on foot or by vehicle, plus a valid driver license and ability to travel within the Portsmouth/Hampton Roads region.
