Salary Range: $24.00 - $26.00 per hour, DOE

What You'll Do: The Impact You'll Make

As an Outpatient Case Manager, you will provide comprehensive case management services to adults with a full spectrum of mental health diagnoses. Working closely with clinicians and community partners, you will help clients access essential resources, develop independent living skills, and navigate their recovery journey through compassionate, person-centered care.

Case Management & Client Support

• Coordinate medical follow-up appointments and support clients with co-occurring medical and behavioral health needs.
• Provide life skills training that promotes independence, wellness, and long-term recovery.
• Assist clients in securing and maintaining stable housing by identifying and coordinating appropriate housing resources.
• Connect clients with community-based services, benefits, and supportive resources to address individual needs.
• Provide crisis intervention and stabilization services while supporting clients through challenging situations.

Documentation & Compliance

• Complete timely, accurate, and thorough documentation in accordance with Los Angeles County Department of Mental Health (LAC-DMH), Medi-Cal, and agency standards.
• Maintain required productivity expectations while ensuring compliance with all applicable regulations and documentation requirements.
• Maintain confidential client records and support quality assurance efforts.

Team Collaboration & Service Delivery

• Work collaboratively within a multidisciplinary treatment team to coordinate comprehensive client care.
• Participate in team meetings, case conferences, and treatment planning.
• Follow established agency policies, procedures, and clinical protocols while managing an independent caseload.
• Support continuous quality improvement initiatives and contribute to the success of the outpatient program.

What You'll Bring: Your Qualifications

Education and Experience

• Bachelor's degree in Psychology, Social Work, Counseling, or a related behavioral health field required.
• Experience working with adults experiencing serious mental illness preferred.
• Experience with Los Angeles County Department of Mental Health (LAC-DMH) documentation and procedures is highly preferred.
• Military-experienced candidates are strongly encouraged to apply.

Skills and Abilities

• Strong interpersonal, communication, and relationship-building skills.
• Ability to effectively engage individuals from diverse cultural and socioeconomic backgrounds.
• Excellent organizational skills with the ability to manage multiple priorities and meet documentation deadlines.
• Proficiency with electronic health records and basic computer applications.
• Ability to work independently while collaborating effectively within a multidisciplinary team.
• Bilingual (Spanish/English) is preferred but not required.

Licenses and Requirements

• Valid California Driver's License with an acceptable driving record.
• Must be insurable under the agency's automobile insurance policy.
• Successful completion of a Live Scan background check is required.
