Position: OUTPATIENT CASE MANAGER

 

Reports To: CASE MANAGER SUPERVISOR

 

 

Position Summary:

 

The Case Manager will coordinate client services per the treatment plan for the adult clientele. Each case manager will be assigned specific clients at the time of intake and be required to program and monitor such things as court appearances, doctor appointments, family therapy sessions, and discharge referrals.


Essential Duties and

Responsibilities:

 

  1. Maintain a caseload of active clients who require case management in such areas as programming court appearances, doctor appointments, family therapy sessions, and discharge referrals.
  2. Act as liaison between courts, families, and outside agencies.
  3. Provide case management services in accordance with the service plan as appropriate to the program.
  4. Coordinate and meet with the members of the Treatment Team and provide them the appropriate reports prior to meeting.
  5. Completes documentation and reports as necessary.
  6. Provides on-call services as needed.
  7. Attends staff meetings as necessary and participates as a team member.
  8. Upholds total confidentiality with respect to clients in all instances.
  9. Ensure that each client is registered for food stamps. Follow up on the completion of the process, working jointly with administrative Case Manager.
  10. Maintain open communication with family, courts, and others as necessary.
  11. Ensure that authorization for Medicaid clients is in on time.
  12. Monitor the billing data/units for Medicaid.
  13. Maintain client case management logbook in accordance with Program/Contract guidelines.
  14. Attends training and in-services as required.
  15. Performs other duties as appropriately and assigned.

Education and/

or Experience: Bachelor’s degree in psychology, social work, mental health or related field. One year’s experience working in the mental health field serving diverse population. Possess case manager certification or obtain certification within three months of employment. Maintain a active driver license.


Reasoning Ability: Ability to apply common sense understanding to carry out instructions furnished in written or oral form.
