Job Summary:

The Division of General Internal Medicine at the University of Colorado School of Medicine is seeking a Patient Care Coordinator to identify patients not reaching healthcare goals to provide outreach and direct patient communication under the direction of the Medical Director. This position will be split between the Lowry and Cherry Creek Internal Medicine Clinics, ambulatory primary care practices that care for patients age 18 and older with complex medical conditions. This is an exciting opportunity to provide top-shelf patient care, particularly for candidates that are interested in gaining experience in the healthcare industry.

Key Responsibilities:

  • Run and interpret patient reports to target patient outreach.

  • Conduct patient outreach calls regarding preventive care, chronic disease management (diabetes, hypertension etc.) and accessing the health care system.

  • Work collaboratively with population health team, nurses, and medical assistants in a team-based approach to meet the needs of a patient.

  • Assist patients in following-through with their treatment plan and document efforts in their electronic health records.

  • Obtain outside records for improved communication and documentation concerning a patient’s care.

 

Work Location:

Hybrid – This role is eligible for a hybrid schedule of 1-2 days per week in each clinic (2-4 days overall) and as needed for in-person meetings.

Qualifications:

Minimum Qualifications:

Entry Level:

  • Bachelor’s degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, public health or a directly related field from an accredited institution.

 

Intermediate Level:

  • Bachelor’s degree in biological science, nutrition, nursing, health, human services, psychology, counseling, social work, public health or a directly related field from an accredited institution.

  • One (1) year of professional level experience in health-related field.

 

Substitution: A combination of education and related technical/military/paraprofessional experience may be substituted for a bachelor’s degree on a year for year basis.

Applicants must meet minimum qualifications at the time of hire.

Preferred Qualifications:

  • Experience in ambulatory healthcare.

  • Experience working as part of an interdisciplinary healthcare team.

  • Experience with electronic medical records, Epic preferred.

  • Customer service experience.

Knowledge, Skills and Abilities:

  • Excellent computer literacy including experience with Microsoft Office Suite (including Excel, Word, PowerPoint, and Outlook).

  • Basic knowledge of electronic medical records systems (i.e. Epic).

  • Excellent communication skills and enjoy interacting with patients, health care providers, and care teams.

  • Able to simultaneously speak with a patient or provider and take notes.

  • Some knowledge of medical terminology, preventative procedures, etc.

  • Able to work with minimal supervision.

  • Creative and proactive in taking initiative.

  • Professional, responsible, and persistent.

  • Able to think critically and find solutions to a variety of problems.

  • Comfortable working with patients experiencing difficulties or barriers.

Anticipated Pay Range:

The starting salary range (or hiring range) for this position has been established as:

Entry Level: $60,257 – $80,744

Intermediate Level: $65,574 – $87,869

The above salary range (or hiring range) represents the University’s good faith and reasonable estimate of the range of possible compensation at the time of posting. This position may be eligible for overtime compensation, depending on the level.
