The Ohio Department of Medicaid (ODM) is committed to improving the health of Ohioans and strengthening communities and families through quality care. In 2020, ODM introduced a new vision for Ohio's Medicaid program - one that strengthens Ohio's future and ensures everyone has the chance to live life to its full potential.

 

OhioRISE, or Resilience through Integrated Systems and Excellence, is Ohio’s first highly integrated care program for youth with complex behavioral health and multi-system needs. Care Management Entities (CMEs)  are vital to the success of the OhioRISE model and will each serve a separate catchment area – part of a county or multiple counties that make up their geographic footprint.

Summary:

As a partner in building brighter futures for youth and their families, your role will be to assist with and facilitate administrative duties for your assigned program that align with The Village Network’s Mission, Vision & Values.

 

Essential Tasks, Duties, and Responsibilities:


Orient TVN OhioRISE employees to onboarding and training requirements for position; In collaboration with direct supervisors, assist new employees through the initial training phases of OhioRISE for the first 180 days of employment.
Schedule opportunities to assure transfer of knowledge from OhioRISE classroom training to practice setting; Model and train desired best practices by cofacilitating activities in home and community settings.
Remain up to date on most current best practices and facilitate ongoing training in care coordination best practices for TVN OhioRISE workforce; Provide follow up training for topics covered by Aetna and the Center of Excellence.
Collaborate with Program leadership to create, implement and update a standard initial and ongoing training curriculum for OhioRISE.
Schedule, Coordinate and Provides internal and hands-on training on care coordination best practices; Implement various training methods to enhance employee’s skills and performance; Evaluate employee learning styles for the creation of meaningful training sessions
Schedule, coordinate and provide refresher courses in OhioRISE related topics
Provide hands-on training and coaching in the home and community settings with new hires during the first year of employment; Serve as a mentor for new hires in learning both OhioRISE best practices and adopting TVN’s 6-R culture; Break down complex tasks into manageable task to allow for skills building
Assesses individual training needs for each employee and collaborates with employee and direct supervisor to create an individualized professional growth and development plan; Provide remedial training as needed
Track employee participation in completion of OhioRISE training requirements and yearly QMHS training requirement. Utilize LMS to document tracked trainings and acquired skills
Develop and implement training evaluation system for continuous quality improvement in training methods and curriculum
Provide and present annual impact report
Performs other duties as required.


 

Knowledge, Skills, and Abilities:


Bachelor’s or Master’s degree in social work or related area. 
Licensure preferred
Have a background and at least 5 years’ experience in one or more of the following areas of expertise: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development is required
Be culturally competent or responsive with training and experience necessary for training family engagement coordinators to manage complex cases and ensures the ability to work respectfully and effectively with people of all cultures, classes, races, genders, ethnic backgrounds, religions, sexual orientations, mental and physical abilities, ages and appearances in a manner that recognizes, affirms and values the worth of individuals, families and communities and protects and preserves the dignity of each. 
Ability to use a computer; proficiency in Word and Electronic Health Record (EHR). 
Valid Ohio Driver’s License and maintains a driving record that allows that individual to be insurable with the insurance company providing The Village Network with vehicle insurance. 
Willingness to travel for various reasons, mainly during the day but occasionally overnight.  Ability to visit clients’ homes (may or may not be handicapped accessible).
Excellent verbal and written communication skills; strong teamwork and organization/time management skills.


 

Physical Demands:


Occasionally move about inside the office to access file cabinets, office machinery, etc.
Constantly operates a computer and other office productivity machinery, such as a copy machine, computer printer, etc.
Ability to effectively communicate (orally and written) with all levels of personnel, internal and external to the company.
Ability to handle bending, stooping, lifting, pushing, reaching, and walking for periods of time.
Must be able to lift 20 pounds independently
