JOB DESCRIPTION

Position: Patient Care Coordinator-Mobile

Hours: Part-time, Non-Exempt

Location: Throughout Los Angeles County (homebase - West Los Angeles)

Reports to: Patient Services Director

General Description:

The Patient Care Coordinator is responsible for coordinating the off-site Mobile Medical Unit (MMU) operations and will work with community partners and Claris staff to schedule off-site clinic services, food and supply distributions and occasional mobile tours for fundraising and church events. The Patient Care Coordinator will provide advocacy, case management and referral services. Other duties include scheduling appointments, receiving and directing visitors, researching and visiting potential new mobile sites and maintaining patient records and clinic log books.

Qualifications:

  • Agree with and uphold the Claris Health Mission Statement, Core Values, and Policies and Procedures.
  • Bilingual Spanish is preferred.
  • Express a desire to help women and families in unintended pregnancies and will not refer or advise a woman to have an abortion.
  • Excellent oral and written communication skills.
  • Detail-oriented and highly organized.
  • Possess excellent interpersonal skills, particularly the ability to work with groups and organizations, both internal and external to the corporation.
  • Possess the ability to provide leadership and support to other staff members and volunteers.
  • Preferably hold at least a Bachelor’s Degree in counseling or social work and have at least two years relevant work experience in advocacy, patient care and management.
  • Medical Assistant and phlebotomy skills are preferred but not required.

Responsibilities:

PATIENT CARE

  1. Provide oversight and facilitate coaching, case management, and advocacy sessions with patients. This includes initial and follow up visits.
  2. Work closely with medical staff to ensure proper follow-up with all medical patients.
  3. Oversee all mobile volunteers and serve as a resource and training support.
  4. Work with Patient Care advocates, Parenting Program staff, and Therapists at each Claris Health Clinic to ensure appropriate referrals and resources.
  5. Ensure that patients receive comprehensive and appropriate educational material, relating to their specific needs.

ADMINISTRATION

1. Conduct annual volunteer reviews

2. Consistently update the policies and procedures related to the Mobile Medical Unit. This includes daily opening and closing procedures, patient care, case management, and all position-related manuals, logbooks, and patient materials.

3. Maintain patient records in the EHR system and all related MMU logbooks (maintenance, equipment, care, etc.)

4. Maintain statistics for grant-related data, appointment demographics, and quarterly Board Reports, and anything else needed to highlight the position.

5. Assist drivers in communicating with maintenance facilities for repairs, regular maintenance, and other vehicle concerns.

QUALITY ASSURANCE

  1. Conduct quality assurance evaluations, risk assessments and non-scheduled checks to ensure proper implementation of AAAHC requirements and Claris policies and procedures.
  2. Ensure that all forms and documents are up to date, approved by the board or other appropriate representatives, and in compliance with agency and industry standards.
  3. Conduct on-site safety orientations for new staff members.
  4. Conduct safety drills for on-site staff members.

COMMUNITY RELATIONS

  1. Represent Claris Health’s patient services within the local community and at special events, fundraising events, church fairs, mobile clinic tours etc..
  2. When called upon, make presentations on topics such as patient advocacy, sharing patient testimonies at fundraising events or church fairs.
  3. Interface with other community agencies to build relationships to better serve various client needs.
  4. Maintain relationships with all current mobile site contacts as well as establish new site contacts.

HUMAN RESOURCES

1. Oversee the training (phone scripts, intake, counseling, etc.) and onboarding of new MMU volunteers.

2. Provide direct supervision of all mobile administrative volunteers.

3. Assist HR personnel with the onboarding of any MMU volunteers.

Physical Demands:

While performing the duties of this job, the Patient Care Coordinator is regularly required to sit, stand, walk, speak and hear. This position requires extensive computer use so the employee must have sufficient hand dexterity to use a computer keyboard and be capable of reading a computer screen. The employee must occasionally lift and/or move up to 20 pounds. Reasonable accommodations may be made to enable otherwise qualified individuals with disabilities to perform the essential functions.

Travel:

The Patient Care Coordinator must be able to travel between Claris Health locations and mobile clinic sites and to attend conferences, training, and other events as required in order to acquire and maintain proficiency in fulfilling the responsibilities of the position.

Pay Transparency Statement
The annualized base salary ranges for the primary location and any additional locations are listed below. Claris Health’s pay ranges vary based on work location. As part of the total compensation package, this role may be eligible for the Workday Bonus Plan or a role-specific commission/bonus, as well as annual refresh stock grants.
