Job Summary

The Administrative Coordinator for Social Work, Case Management & Utilization Department provides administrative and operational support to the hospital's Social Work, Case Management & Utilization Department. This role ensures efficient coordination of patient care activities, supports case managers and social workers, and assists with documentation, communication, scheduling, appeals, performance improvement initiatives, and data tracking to facilitate timely and effective patient care transitions.

Essential Functions
  • Provide administrative support to the SW/Case Management team, including case managers, social workers, and department leadership.
  • Coordinate and schedule multidisciplinary meetings, patient care conferences, and discharge planning meetings.
  • Maintain accurate patient case records, logs, and departmental databases in accordance with hospital policies and HIPAA regulations.
  • Assist with patient discharge planning by coordinating communication with external facilities such as rehabilitation centers, skilled nursing facilities, home health agencies, and insurance providers.
  • Prepare reports, spreadsheets, and departmental metrics related to case management activities, patient flow, and discharge planning.
  • Manage incoming calls, emails, and inquiries related to case management services.
  • Track authorizations, referrals, and follow-up documentation to ensure compliance with regulatory and payer requirements.
  • Assist in maintaining department policies, procedures, and forms.
  • Coordinate office operations including supply management, scheduling, and document organization.
  • Support quality improvement initiatives and audits by organizing documentation and tracking performance metrics.

Qualifications
  • AA
  • 4-6 years Experience working with electronic medical records (EMR) systems preferred.
  • Strong organizational and multitasking abilities
  • Excellent written and verbal communication skills
  • Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint)
  • Knowledge of healthcare terminology and hospital workflows
  • Ability to maintain confidentiality and comply with HIPAA regulations
  • Strong attention to detail, problem-solving and critical thinking skills
  • Ability to collaborate with multidisciplinary healthcare teams
  • Proficiency in EPIC, Allscripts/Careport systems
