Summary & ResponsibilitiesAn Assertive Community Treatment (ACT) team consists of a community-based group of medical, behavioral health and rehabilitation professionals who use a team approach to meet the needs of an individual with severe and persistent mental illness. ACTT is a team dedicated to reducing and eliminating the symptoms of severe mental illness and to enhance the individual's quality of life. The purpose of the AP/QP Generalist is to serve as a therapeutic resource to consumers as a member of an established Assertive Community Treatment Team (ACTT)

 

Job Duties include:


Serves as an active member of the ACT Team, participates in team meetings, and provides input into the person-centered planning process
Show knowledge and use of clinical process to achieve positive outcomes to include: evaluation, assessment, service, treatment planning, intervention techniques, aftercare, and transition planning
Remain available face to face or by telephone, 24 hours a day 7 days a week as staff serves as the first responder in the event of crisis situations. The service site will be at any location the consumer or family needs
Effectively use clinical and administrative supervision to improve to quality and performance of services for consumers and organizations
Demonstrate the ability to manage an assigned caseload and meet the direct care billable productivity expectations of the organization


 

Basic Requirements:


Ability to perform tasks on the computer including working with Microsoft 360, Excel, Word and other basic programs.
Valid driver’s license, auto insurance coverage and reliable transportation
Pass criminal background check and other regulatory checks
Provide proof of your education


 

Education and Experience Required:


A bachelor’s degree in Special Education, Psychology, Social Work or closely related Human Services area
 Two (2) years of professional MH work experience with adults
Exceptional written, oral, interpersonal, and organizational skills.
High level proficiency in Microsoft Office- MS Word, Excel, Access, and PowerPoint.
