Meds Only Case Manager

We have an immediate Opening for a Meds Only Case Manager.

Key Responsibilities:

• This individual will be working directly with children and their families in the community.
• This individual should be familiar with Targeted Case Management and be highly organized to complete assessments, care plans,  and monitor the patient's needs.
• Flexible work schedule is required for this position to ensure continuity of care.

Required Skills/Education:

• Bachelors or Master’s Degree in human services, social work, or counseling to work with children 0-18 and their families in the home, community, and school.
• Needs to have experience working with systems, understand family dynamics, behavior management strategies, and work well with a treatment team.
• Strong organizational, liaison, and advocacy skills necessary.
• Prior experience in a mental health, human service, or school setting a plus.
• The Targeted Case Manager must hold a valid driver’s license and be able to demonstrate a clean driving and criminal record.
