The Impact You’ll Make

  • Direct Counseling & Support: Provide individual counseling, crisis intervention, and relapse prevention strategies tailored to patients undergoing medication-assisted treatment.

  • Whole-Person Assessment: Utilize motivational interviewing and comprehensive psychosocial evaluations to identify barriers to care, social determinants of health, and individual recovery goals.

  • Collaborative Care Coordination: Partner seamlessly with our medical and behavioral health teams to manage patient registries, track progress, and ensure smooth transitions across levels of care.

  • Resource Navigation: Connect patients and their families with essential community services, housing resources, peer support networks, and financial assistance programs.

  • Quality & Continuous Improvement: Participate in team huddles, quality improvement initiatives, and data tracking to ensure our MAT program continues to meet the highest standards of care.

Who You Are

  • Licensed & Qualified: You hold an active, unencumbered professional license in the State of Vermont (e.g., LCMHC, LICSW, or LDAC/CADC equivalent) within your professional scope of practice.

  • Experienced in Addiction Treatment: You have a strong foundation in substance use disorder (SUD) treatment, harm reduction principles, and trauma-informed care.

  • A Compassionate Communicator: You excel at building trusting, non-judgmental relationships and utilize motivational interviewing with ease.

  • A Collaborative Problem Solver: You thrive in a fast-paced, multidisciplinary team environment and know how to advocate effectively for your patients.
