Part time - 24 hours / week. Field position with a blended caseload of nursing home and home patients. Mostly greater Nashua area. Occasional weekend day. 

As a Medical Social Worker, you’ll play a vital role in helping patients and families navigate the emotional, social, and practical challenges that can accompany illness, injury, and end-of-life care. Working as part of an interdisciplinary Home Health or Hospice team, you’ll provide compassionate support, connect patients with valuable community resources, and help ensure care plans address the whole person.
What You’ll Do:

  • Conduct comprehensive psychosocial assessments to identify patient and family needs.
  • Develop and implement social work interventions that support patient goals and the overall plan of care.
  • Connect patients and families with community resources, financial assistance programs, and supportive services.
  • Collaborate closely with physicians, nurses, therapists, and caregivers to coordinate care and discharge planning.
  • Facilitate family meetings and provide guidance through complex medical and emotional situations.
  • Advocate for patients and assist with benefit applications and other available resources.
  • Maintain timely, accurate documentation within the electronic medical record.
  • Participate in case conferences, staff meetings, educational programs, and community outreach activities.
  • Provide exceptional customer service and patient-centered care.

Who You Are:

  • Bachelor’s degree in Social Work required.
  • Master’s degree in Social Work required for Homecare positions in accordance with Medicare regulations.
  • Minimum of two years of social work experience in a healthcare setting.
  • Experience working with older adults and families.
  • Home Health, Hospice, Palliative Care, or End-of-Life Care experience preferred.
  • New Hampshire social work licensure preferred.
  • Strong assessment, counseling, advocacy, and care coordination skills.
  • Valid driver’s license with an acceptable driving record.
