Job DetailsJob Location: Onslow Memorial Hospital - Jacksonville, NC 28546Position Type: Full Time 1.0Job Shift: EveningJob Category: OtherThe purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, care coordination, and therapeutic support to patients and families within the hospital setting. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and discharge planning. Collaborates with the multidisciplinary care team to identify patients at risk for readmissions and implements plans of care to reduce risk. The Inpatient Case Management Social Worker must be highly organized professional with great attention to detail, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies.

 

Position can either be 5 days a week from 1400 to 2200 OR 4 days a week from 1400 to Midnight.
QualificationsEducation/Certification:

Master's degree in Social Work (MSW) from a program accredited by the Council on Social Work Education required and a License in Clinical Social Worker or a License in Clinical Social Worker Associate.

 

Experience:

Two (2) years of medical care management experience as a Social Worker preferred.
