Medical Social Worker – Hospice

Location: Lancaster, OH and surrounding communities
Employment: Full-time
Hours: 8:30 AM–5:00 PM
On-Call: Required rotation, including weekends and holidays
Position Type: This position requires driving between patients' homes and facilities

We are looking for a Medical Social Worker to serve patients throughout Lancaster, Ohio and the surrounding area. This position provides an opportunity to build meaningful relationships with patients and families while working as part of a collaborative hospice care team.

The role is primarily field-based, with visits taking place in patient homes and healthcare facilities.

Your Role

As a member of the hospice team, you'll help patients and families identify their needs and connect them with appropriate support and resources.

Your responsibilities will include:

  • Evaluating the psychosocial needs of patients and their families

  • Developing and providing appropriate social work interventions

  • Offering emotional support, counseling, and crisis assistance

  • Helping families navigate community resources and available services

  • Providing guidance with advance care planning and related concerns

  • Supporting patients and caregivers as they cope with changes associated with serious illness

  • Assisting families with anticipatory grief and bereavement needs

  • Advocating for patient preferences, dignity, and quality of life

  • Recognizing and responding to cultural, emotional, social, and spiritual considerations

  • Participating in interdisciplinary care planning with nurses, physicians, chaplains, and other clinicians

  • Documenting assessments, visits, interventions, and follow-up in a timely manner

  • Traveling throughout the assigned service area to meet with patients

  • Participating in the organization's on-call, weekend, and holiday rotation

Qualifications

Required

  • Bachelor's degree in Social Work

  • Active Ohio LSW or LISW license

  • At least one year of social work experience in a healthcare environment

  • Valid driver's license

  • Reliable personal transportation

  • Acceptable driving history

  • Automobile insurance meeting state requirements

  • Ability to travel between patient homes and facilities

  • Comfortable using electronic systems for clinical documentation

Additional Experience We Value

  • MSW or other Master's degree in Social Work

  • Previous hospice or palliative care experience

  • CHP-SW or ACHP-SW certification

  • Current CPR certification

  • Notary designation or willingness to obtain one within six months of starting
