Are you a Social Worker looking for meaningful work where you can make a direct difference in the lives of patients and families?
We are seeking a Medical Social Worker to join our hospice team serving Columbus and surrounding Ohio communities. In this field-based role, you’ll support patients and families as they navigate the emotional, social, and practical challenges associated with serious illness and end-of-life care.
You’ll work closely with an interdisciplinary team of nurses, physicians, chaplains, and other healthcare professionals to help patients receive compassionate, individualized care.
Location: Columbus, OH and surrounding communities
Schedule: Full-time, Monday–Friday, 8:30 AM–5:00 PM
On-Call: Required rotation, including weekends and holidays
Work Setting: Field-based – patient homes and healthcare facilities
What You’ll Do
Complete psychosocial assessments for hospice patients and families
Provide counseling, emotional support, and crisis intervention
Help patients and families access community resources
Assist with advance care planning and other practical needs
Address social, cultural, spiritual, and emotional concerns
Support patients and families experiencing anticipatory grief
Provide education and support related to end-of-life care
Advocate for patient and family needs and preferences
Collaborate with nurses, physicians, chaplains, and other members of the hospice team
Maintain accurate and timely clinical documentation
Participate in the required on-call rotation
Provide coverage during scheduled weekends and holidays
Travel between patient homes and healthcare facilities throughout the service area
What You Bring
Required Qualifications
Bachelor's degree in Social Work
Current Ohio Social Work license (LSW or LISW)
At least 1 year of social work experience in a healthcare setting
Valid driver's license
Reliable transportation and acceptable driving record
Required automobile insurance coverage
Strong communication, computer, and documentation skills
Ability to travel throughout the assigned service area
Preferred Qualifications
Master's degree in Social Work (MSW)
Hospice or palliative care experience
CHP-SW or ACHP-SW certification
CPR certification
Notary designation or willingness to obtain within 6 months of hire
