Are you a Social Worker looking for meaningful work where you can make a direct difference in the lives of patients and families?

We are seeking a Medical Social Worker to join our hospice team serving Columbus and surrounding Ohio communities. In this field-based role, you’ll support patients and families as they navigate the emotional, social, and practical challenges associated with serious illness and end-of-life care.

You’ll work closely with an interdisciplinary team of nurses, physicians, chaplains, and other healthcare professionals to help patients receive compassionate, individualized care.

Location: Columbus, OH and surrounding communities
Schedule: Full-time, Monday–Friday, 8:30 AM–5:00 PM
On-Call: Required rotation, including weekends and holidays
Work Setting: Field-based – patient homes and healthcare facilities

What You’ll Do

  • Complete psychosocial assessments for hospice patients and families

  • Provide counseling, emotional support, and crisis intervention

  • Help patients and families access community resources

  • Assist with advance care planning and other practical needs

  • Address social, cultural, spiritual, and emotional concerns

  • Support patients and families experiencing anticipatory grief

  • Provide education and support related to end-of-life care

  • Advocate for patient and family needs and preferences

  • Collaborate with nurses, physicians, chaplains, and other members of the hospice team

  • Maintain accurate and timely clinical documentation

  • Participate in the required on-call rotation

  • Provide coverage during scheduled weekends and holidays

  • Travel between patient homes and healthcare facilities throughout the service area

What You Bring

Required Qualifications

  • Bachelor's degree in Social Work

  • Current Ohio Social Work license (LSW or LISW)

  • At least 1 year of social work experience in a healthcare setting

  • Valid driver's license

  • Reliable transportation and acceptable driving record

  • Required automobile insurance coverage

  • Strong communication, computer, and documentation skills

  • Ability to travel throughout the assigned service area

Preferred Qualifications

  • Master's degree in Social Work (MSW)

  • Hospice or palliative care experience

  • CHP-SW or ACHP-SW certification

  • CPR certification

  • Notary designation or willingness to obtain within 6 months of hire
