Home Health Medical Social Worker (MSW) — PRN

Preferred Care Home Health is seeking a compassionate, experienced PRN Medical Social Worker to support patients and families navigating health challenges in the comfort of their own homes. This is a flexible, per-diem opportunity ideal for an MSW who wants meaningful, patient-centered work without a full-time commitment.

As part of our home health team, you'll help patients cope with the emotional, social, and practical realities of illness or disability — connecting them to resources, supporting care planning, and empowering them to live as independently and fully as possible.

What You'll Do

Support Patients & Families

• Assess the social and emotional factors affecting a patient's health and recovery
• Help patients understand, accept, and follow their physician's care recommendations
• Guide patients and families through personal and environmental challenges that impact healing
• Provide services aimed at restoring patients to their optimum level of well-being

Coordinate Care & Connect Resources

• Contribute to the patient's Plan of Care as part of the interdisciplinary team
• Identify and connect patients with community agencies, family support, and other resources
• Support discharge planning and serve as a consultant to fellow health care staff

Document & Collaborate

• Complete clinical and progress notes within required timeframes
• Participate in team conferences, in-services, and Performance Improvement initiatives
• Uphold strict confidentiality for all patient, employee, and Agency information

What You Bring

• Education: Master's degree in Social Work from a CSWE-accredited program (required)
• Experience: Minimum of 1 year of social work experience in a health care setting (home health experience a plus)
• License: Florida social work license preferred, not required
• Skills: Strong assessment, communication, documentation, and care coordination abilities; comfortable managing multiple priorities and working independently
• Mindset: Self-motivated, resourceful, and genuinely people-focused
