Rate of Pay:  $29.47 to $33.60
Bonus: $5,000 with 2-year commitment

Responsibilities

  • Assists clients in navigating inpatient medical stays.
  • Provide support for patients to experience culturally appropriate care while in an inpatient setting
  • Assist Clients in obtaining Durable Medical Equipment (DME).
  • Accept, record and ensure completion of referrals from other providers outside the agency.
  • Conduct intake interviews with clients.
  • Develop a plan of care with client/families when appropriate.
  • Offer options within network when appropriate.
  • Identify and secure alternative resources for clients.
  • Provide follow-up services to client/family.
  • Participate in inter-professional team-based primary care casework analysis and planning.
  • Exchange pertinent information with team members as necessary.
  • Provide feedback to referring provider when appropriate.
  • Coordinate placement in rehabilitation programs such as discharge after hospitalization or treatment facilities for substance abuse when necessary.
  • Assist with discharge planning from the hospital using a shared decision-making approach with patients and families to support safe transitions to home, rehabilitation facilities, assisted living, or long-term care as appropriate.
  • Assist patients with obtaining insurance coverage for long term care and helping them understand the policy’s coverage.
  • Assist with coordinating patient care conferences. Work with staff to schedule care conferences with providers, outside agencies and appropriate family members or POA if applicable. Referring patients to the FDL HSD Patient Advocates or care coordinators to assist in obtaining other insurance coverage.
  • Assist in completing forms requiring a provider’s signature, including but not limited to Physician Orders for Life-Sustaining Treatment forms, disability and medical necessity forms, medical opinion forms, school and childcare health forms, insurance and coverage forms, workers’ compensation documents, FMLA forms, Durable Medical Equipment requests, and prescription-related forms.
  • Participate in patient care conferences and serve as the patient advocate by identifying unmet needs and helping develop strategies to address them. .
  • Coordinate care and shelter services for people facing homelessness, sexual violence or other significant challenges.
  • Assist with reporting abuse/neglect to the appropriate authorities.
  • Coordinate patient care with inpatient care facilities not associated with FDL.
  • Coordinate patient care with the FDL Social Services or county social services.
  • Coordinate home care needs with FDL Home Care or other home care providers.
  • Assist patients with advanced care directives upon request.
  • Assist with Medical Assistance restricted patient requirements/changes.
  • Connect veterans to FDL Veteran Service Office (VSO) or county VSO; provide education to patients and families on alternative community resources.
  • Assist medical department leadership in identifying unmet client needs and ways of addressing those needs.
  • Provide interdepartmental collaboration across all HSD departments, including medical, community health, social services, dental, behavioral health, and the substance use department.
  • Provide and support inter facility collaboration across all spectrums of the healthcare continuum.
  • Maintain awareness of financial and medical resources in the service area.
  • Stay informed about the county, state, federal, educational, vocational, and therapeutic programs for Indian people.
  • Document client services rendered in the electronic health record.
  • Appropriate work attire is required.
  • Must attend all mandatory department training courses.
  • Ensures confidentiality of financial records and all records for employees, band members and/or clients.
  • Displays a responsive and professional manner in promptly responding to all requests, complaints, and problems.
  • Recognizes that each employee is a representative of the Fond du Lac Band of Lake Superior Chippewa and is responsible for demonstrating courtesy, respect, and sensitivity to the needs of all others, including visitors and co-workers.
  • Represents the Fond du Lac Band of Lake Superior Chippewa and the department in a positive and professional manner in the community.
  • Required to maintain proper attendance including reporting to work on time in accordance with applicable policies.
  • Maintains a clean and organized work area.
  • Due to changes and modifications in the job from time to time, employees are required to be flexible and assume other responsibilities assigned by management as deemed necessary.

Qualifications

  •  Valid Driver’s License issued in the US is required.
          o Driving eligibility determined by FDL approved guidelines.
  • A master’s degree in social work required
  • Medical background is desired.
  • Experience in an in-patient/hospital setting is desired
  • Ability to communicate effectively orally and in writing is required.
  • Ability to work independently and establish priorities is required.
  • Attention to detail and accuracy is required.
  • Ability to establish professional and harmonious working relationships on all projects and with all parties involved.
  • Subject to drug and alcohol testing in accordance with the Fond du Lac Reservation’s Personnel Policies.
  • Subject to pre-employment and annual background checks.
  • Subject to immunization review and submitted proof of required vaccinations.
  • Travel is required.
