Job Summary/Purpose 

To work closely with hospital discharge planners, physicians, palliative care team members, social workers and RN unit staff to assist with admitting patients to the Ohio’s Hospice program and to be available as a resource for facilitating the safe transfer to ECF, Home or Hospice Care Center. The Social Work liaison will collaborate with the Ohio’s Hospice Admissions and Referrals Department and the Care Team Coordination Department.

Key Responsibilities 

• To work with patients/families with complex psychosocial issues

To engage in a thorough discussion of patient/ family goals of care, education on how hospice can help meet those goals and as needed education on other community resources also available to assist

To provide emotional support for the crisis sometimes created when end of life issues is presented to the patient/family.

To educate regarding care setting alternatives including home, long term care, skilled nursing facility, private duty caregiving, and hospice houses

To ensure the understanding of the Hospice benefit (informed consent)

To address insurance or lack of insurance concerns and complete Financial Assessments as appropriate

• To address Advance Directives needs

• To communicate with hospital staff to establish a coordinated plan of care that reflects the hospice philosophy for both new patients and existing Ohio’s Hospice patients that are in the hospital; which will ensure that transfers to other care sites are expedited as soon as possible.

• To assist with obtaining Consents

• To provide an initial and on-going comprehensive psychosocial assessment of patients in facility based contracted beds

• To provide social work case management to Ohio’s Hospice patients admitted to the hospital for symptom management


What You Need to Know about the Medical Social Worker Liaison Role: 

  • This is a Part-Time role, with the hours of: 8AM-5PM.
  • This position is based out of our in-patient unit at Ohio State University Wexner Medical Center but will require travel to our inpatient care center at Licking Memorial Hospital as well.
