What You'll Do

As a Medical Case Manager, you'll be a trusted advocate for people living with HIV: Helping them navigate healthcare; access essential resources; and achieve their health goals. You'll work closely with clients, medical providers, and community partners to remove barriers and improve quality of life.

In this role, you will:

  • Build meaningful relationships with clients
  • Develop individualized care plans and connect clients to resources
  • Coordinate care with healthcare providers and community agencies
  • Support clients in staying engaged in medical care and treatment
  • Assist with benefits, health services, and other support programs
  • Maintain accurate documentation and client records
  • Collaborate with a compassionate, client-centered team


Minimum Requirements

  • Bachelor's degree in a health or social service field plus 1-2 years of related experience

              OR

  • 2-3 years of applicable experience in lieu of a degree
  • Valid Arizona Driver's License/Auto Insurance


Bonus Qualifications

  • Bilingual English/Spanish
  • Experience with HIV/AIDS services
  • Knowledge of Ryan White or HOPWA programs
  • Experience with electronic health records
  • Experience in healthcare, social services, behavioral health, housing, or community outreach

Compensation & Benefits
