Easter Seals Greater Houston, a leading provider of services for individuals of all ages and abilities, is seeking an experienced and compassionate Medicaid Waiver Case Manager to join our Community Living Assistance and Support Services (CLASS) team.

Our CLASS Case Management team provides comprehensive case management services to individuals enrolled in the Texas Medicaid Waiver program, helping them access essential supports and services that promote independence and quality of life.

To qualify as a CLASS Program Case Manager in Texas, applicants must meet the education and experience requirements established by the Texas Health and Human Services Commission (HHSC).

Essential Responsibilities

  • Complete mandatory HHSC training, including the CLASS Policy and Process course, and achieve a minimum score of 80% on the final examination.
  • Schedule and conduct initial assessments for individuals applying for CLASS services.
  • Assist individuals and families with Medicaid eligibility processes.
  • Coordinate annual service planning meetings and other required meetings.
  • Monitor participant services and eligibility requirements.
  • Complete monthly billing and required documentation.
  • Maintain accurate, complete, and current records, including assessments, evaluations, budgets, authorizations, progress notes, and billing documentation.
  • Conduct monthly contacts and quarterly monitoring visits in home, work, and school settings.
  • Attend required initial and ongoing training, staff meetings, team meetings, and community-based meetings.
  • Collaborate effectively with internal teams, community partners, service providers, and other stakeholders.
  • Support the mission of Easter Seals Greater Houston and perform additional duties as assigned.

Minimum Qualifications

  • Bachelor's degree in a Human Services field; and
  • Minimum of two (2) years of experience working with individuals with disabilities.
  • OrHigh School Diploma or GED; and
  • Minimum of four (4) years of experience working with individuals with disabilities or in a related field.

Additional requirements:

  • Proficiency with Microsoft Word, Excel, and Outlook.
  • Knowledge of Medicaid services and eligibility processes.
  • Strong organizational, documentation, and communication skills.
  • Ability to work independently and manage multiple priorities.

Preferred Qualifications

  • Bilingual in English and Spanish.
  • Two years of documented experience as a CLASS, DBMD, or HCS Case Manager.
  • Two years of documented experience as a CLASS Direct Service Agency employee or Medicaid Managed Care Provider Case Worker.
