Med Group - Licensed Clinical Social Worker (LCSW)
BJC HealthCare- Shift information: Monday – Friday; 8 am - 4:30 pm with some flexibility to work 8:30 am - 5:00 pm as needed
- Team Dynamic: There are 6 full-time Social Workers on this team.
- Case Load size: 15-25 outreach calls to patients per day. Overall caseload size is around 80.
- Office Location: – Mason Ridge in Chesterfield with the option to work from home after 2-3 weeks of training at Mason Ridge. Some traveling required to primary care clinics in your assigned region is necessary.
- MUST HAVE skills for this position: Must be a fully licensed LCSW (unable to consider provisionally licensed candidates at this time)
- Job responsibilities: Telephonic outreach to patients who have either been recently discharged from ED/IP, referred by primary care provider or ACO team member, or patients with complex care needs to comprehensively assess needs, develop care plan, assist with connections to needed resources, educate to help patients become independent managing their health and navigating the health care landscape. To be successful in the role, a background in medical/hospital social worker is VERY helpful. In addition to basic understanding of medical terminology and chronic health conditions is also important.

Role Purpose
Provides clinical social work services to address the psychosocial needs of patients and families. Provides individual and group counseling, linkage to community resources, expertise in cultural diversity and advocacy. Collaborates with the health care team and ensures timely, cost-effective discharge and transition planning.
