SUMMARY AND ESSENTIAL FUNCTIONS:


  • The LISCW or BHMSW will assist psychiatric consultant and primary care providers in recognizing and managing mental health and psychosocial issues and act in a member of the Primary care team. 


  • LISCW or BHMSW will conduct consultation visits that are focused on the referral issues from the health care providers, ideally on the same day as the referral or shortly thereafter. 


  • LISCW or BHMSW will conduct brief, focused interventions (often manualized) for patients in need of mental health services. The emphasis will be on patient education and self-management strategies. Telephone visits may be required at times. LISCW or BHMSW will work closely the psychiatric consultant in patient care through supervision and coordinating treatment plans. 



REPORTS TO: Practice Manager 


SUPERVISES: None


INTERNAL AND EXTERNAL CONTACTS: Medical and Hospital Staff, Patients, Families, Community Resources, Agencies and Clergy. 


QUALIFICATIONS:

  • Excellent working knowledge of behavioral medicine and evidence based treatments for mental health condition. 
  • Ability to work through brief clients contracts as well as make quick and accurate clinical assessments of mental and behavioral health conditions. 
  • Is comfortable with the pace of primary care, working with an interdisciplinary team, and has strong communication skills. 
  • Has good knowledge of psychopharmacology. 
  • Ability to perform consultations and give providers feedback quickly. 
  • Neuropsychological testing experience preferred, but not required. 
  • Psychometric experience preferred, but not required. 
  • Medical Social Work experience preferred. 
  • Counseling skills 
  • Excellent verbal and written communication skills. • 
  • Works with collaborative/team approach. 
  • Organized and is able to prioritize and manage time efficiently. 
  • Must have working knowledge of medical terminology. 
  • Have an understanding of the levels of care and how to move patients along the continuum of health care. 
  • Must be familiar with computer. 


Experience/Specialized Skills:

  • Infants to geriatric (non-specific) determined by referrals. 


  • The LISCW or BHMSW must demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. 


  • The individual must demonstrate knowledge of the principles of growth and development over the life span. 


  • The LISCW or BHMSW must be able to assess and interpret data about a patient’s in order to identify each patient’s age specific needs and provide the care needed by the (neonate, pediatric, adolescent, adult, and geriatric) patient. 



Required Education/Course(s)/Training: Licensed Social Worker or Master’s Level, Licensed Clinical Psychologist (Doctoral) or equivalent. 



Preferred Certification/Registration: Licensed Social Worker- State of New Hampshire
