Hillside Plaza is seeking a compassionate and experienced Licensed Social Worker to help lead and support our Social Services Department. If you’re passionate about advocacy, care coordination, and improving lives in a healthcare setting, we’d love to welcome you to our team.

What You’ll Do:

  • Plan, organize, implement, and evaluate social services programs
  • Ensure compliance with federal, state, and local regulations
  • Support residents and families with emotional, social, and transitional needs
  • Collaborate with interdisciplinary teams to promote holistic care
  • Advocate for residents’ rights and well-being

 

What You Bring:

  • Bachelor’s degree in social work from an accredited school
  • Active LSW license in good standing (Ohio)
  • Minimum 2 years of supervised social work experience in a healthcare setting preferred
  • Long-term care experience preferred
  • Strong communication, empathy, and organizational skills
