Licensed Social Worker - Home Care

Park Nicollet is looking to hire a Licensed Social Worker, (LISW or LICSW), to join our Outpatient Home Care team!  Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.   

Position Summary:

Provides direct social work services to patients, extending services to family members in need of identified social worker services, providing them with psychosocial and environmental support. Makes available a full range of treatment modalities with individuals, families, and groups, based on social work assessment and diagnosis. Coordinates social work services with other departments and providers within the Park Nicollet Health Services i.e., hospice, hospital and clinic-based social workers and out-of-system providers. Provides social work services to the patient and associated community. May provide clinical supervision to other Social Workers. Works with physicians, nurse clinicians, other medical/surgical specialists, and other Park Nicollet Health Services employees. Contact with patients and their families.

Work Schedule: 4 days/week, 8am-4:30pm M-F. 

Required Qualifications:

• Education, Experience or Equivalent Combination:
  • Master’s degree in social work
• Licensure/ Registration/ Certification:
  • Must have Minnesota (MN) Social Work license upon start date: 
    • MN must have one of the following: 
      • Licensed Independent Social Worker (LISW) or 
      • Licensed Independent Clinical Social Worker (LICSW) 
  • Must have vehicle, driver’s license and current auto insurance. Must meet all criteria required under Park Nicollet’s Automobile Liability Guidelines 
• Knowledge, Skills, and Abilities:
  • Requires basic computer/keyboarding skills. 
  • Demonstrates ability to conduct individual and group therapy sessions. 
  • Demonstrates ability to coordinate and facilitate group interactions. 
  • Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients. 
  • Individuals are responsible for the travel arrangements required to meet the expectations of the position.

Preferred Qualifications:

• Education, Experience or Equivalent Combination:
  • Master’s degree in social work 
• Licensure/ Registration/ Certification:
  • MN licensure at the LICSW level.
• Knowledge, Skills, and Abilities:
  • Previous Home Care experience 
  • Advance Care Planning experience 
  • Knowledge of word processing, spreadsheet, and presentation software

Essential Duties: 

• Print Daily Census. Identify high readmissions risk patients.
• Team with RN Care Manager in morning to review patients and assigned work.
• Provide psychosocial assessments and diagnosis which may include differential diagnosis, use of diagnostic tools and application of treatment modalities of patient and family.
• Mobilize and develop individual, family, and community resources in patient care planning and discharge planning to optimize continued health potential of the patient.
• Complete, coordinate and provide continuity of care during the discharge process as it evolves throughout the patient experience, this may be a shared duty with the RN Care Manager.
• Will complete chemical dependency and psychiatric assessments and placements.
• Provide outpatient programs/community resources, (e.g., meals on wheels, alternative care grant, chemical dependency/mental health, etc.)
• Provide financial resources, (e.g.: referral to financial counselors, med-eligible services (MES) MN Drug Card, etc.)
• Assist with Advance Care Planning (ACP) and Provider orders for Life Sustaining Treatment (POLST) completion as needed.
• Coordinate and attend patient care conferences as needed.
• Assessment of involvement in legal components of care to include such tasks as: Child and Adult protection, reports, custody and adoption cases, commitment proceedings, Power of Attorney, and guardian issues.
• Conduct regularly scheduled and ongoing support groups for patients with complex diagnoses and their families as needed.
• Participate in, promote, and direct fund raising activities dedicated to the community of patients. Attend monthly patient family council meetings when are rotate throughout the social worker staff.
• Provide consultation and education to health care teams and selected health care systems regarding psychosocial issues for the patients and their families.
• Performs related administrative tasks related to the position such as insurance authorization reports, statistics and representing social work in the hospital, clinic and community as needed. (e.g.: Care Improvement Teams (CIT), Pre-operative joint class, Caring for Colleagues, etc.)
