How you'll make an impact in this role
- Design Person-Centered Care: Build comprehensive transition plans that bridge the gap between hospital and home, ensuring every step reflects the patient's unique needs and goals.
- Champion Patient Advocacy: Proactively identify and dismantle barriers to care, ensuring your patients have the resources and support systems they deserve.
- Act as a Clinical Subject Matter Expert: Serve as the go-to resource for the care team on complex issues, including crisis intervention, neglect, and adjustment needs.
- Drive Care Excellence: Maintain a continuous, collaborative pulse with supervisors and transition teams to refine health plans and ensure seamless implementation.
What minimum qualifications you'll need
Licensure / Certification / Registration:
- One or more of the following required:
- Social Worker specializing in Clinical Practice obtained prior to hire date or job transfer date.
- Licensed Clinical Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date.
- Preferred Credential(s):
- Drivers License.
Education:
- Master's degree in Social Work required.
Work Experience:
- 3 years of experience preferred.
