With independent or guided direction, the Licensed Professional Counselor (LPC) utilizes counseling and psychotherapy to evaluate and treat emotional and mental problems and conditions, whether cognitive, behavioral, or affective. Working closely with the multi-disciplinary team of MDs, NPs, and PAs, the LPC will conduct comprehensive clinical psychosocial assessments. This position will develop and implement treatment plans and maintain plan updates in electronic medical records, serve as an advocate for and obtain appropriate services for patients. The LPC will work with the client to help identify goals and potential solutions to problems causing emotional turmoil. The counselor also works to improve communication and coping skills, increase self-esteem and promote behavior change and the mental health of their clients. The ideal candidate will have demonstrated knowledge and experience with working with patients and skilled in oral communication, written communication, problem solving/assessment, teamwork, group process facilitation, documentation accuracy, customer service, interpersonal relations, creativity/innovation, adaptability, decision making, and conflict resolution.

Essential Duties and Responsibilities:          

 


Serves as a patient advocate, coordinating with other healthcare providers, community resources, and assists patients to navigate healthcare system and insurance benefits.
Conducts thorough screening, physical and psychosocial assessments by phone or clinical setting; collaborates with patients/families, physicians and other healthcare team members to identify any issues/barriers that affect health condition management and implements a comprehensive patient-centered plan of care.
Develops individualized plan of care, assessing patient's eligibility for needed resources and referring to appropriate team member; collaborates with team members to formulate care plans/goals that meet the need of patient/family; articulates short-term/long-term goals; document accurate/timely information in electronic medical record.
Assists with administration, development, implementation, and evaluation of social work programs and policies.
Provides health education, utilizing a variety of approaches to effectively educate patients/families and other team healthcare members regarding community resources, health care benefits, and insurance and managed care cases; provides health education to assist in disease management and developing coping skills, disease processes, medications, treatment plans and preventative measures.
Monitors patient adherence to treatment goals, identifies barriers to adherence, and promptly revises treatment plans to improve adherence/outcomes
Participates in departmental functions such as staff meetings, staff in-services, and committees, as assigned.
Builds strong rapport with clients and makes accurate assessments of their problems whether mental/emotional or physical.
Reviews and evaluates recent psychosocial health research and interprets it to professionals and patients.
Performs short term counseling and crisis intervention as necessary.
Documents according to departmental standards, Federal and State regulations and can hardcopy medical documents into EMR and label.
Performs all other duties as assigned.


 

Knowledge, Skills, and Abilities:

 


Have knowledge of chronic mental illness and accompanying symptoms
Develops and maintains positive relationships with mental health consumers and their families, the community, other service providers, and with funding sources
Knowledge of medical case management concepts along with care continuum; Medicare benefits; HIPAA and ability to apply during the medical case management process.
Ensures Medicaid policies with regards to services are met and documented appropriately.
Ability to apply clinical knowledge in a medical case management role; understand and comply with regulations and recommendations of outside regulatory bodies with regard to case management services.
Ability to engage and collaborate with patients/families; care/manage a diverse population; prioritize activities and adapt to changing priorities.
Ability to communicate clearly and effectively in emergency situations.
Ability to work with minimal supervision within scope of practice; think critically and analytically; articulate the process of attaining goals and outcomes of medical case management.
Ability to maintain confidentiality
Ability to communicate effectively in oral, written and electronic formats.


 

Typical Working Conditions:

 


Incumbents work in a fast-paced office environment requiring travel to facilities.
Frequent contact with staff, patients, and the public.
Prolonged periods of sitting at a desk and working on a computer in a normal office environment.
Work may be stressful at times and may require occasional evening or weekend work.

This position has access to systems and/or services that contain protected health information (PHI) as defined by the Health Insurance Portability and Accountability Act of 1996.  Additionally, the position ensures and adheres to strict confidentiality when handling patient information, according to the HIPAA Privacy Act and supporting GAPHC policies and procedures.

QualificationsQualifications:

 


Master's Degree in social services field, psychology or closely related field 
Licensure in the State of Georgia as a professional counselor (LPC)
Maintains current CPR certificate.
Demonstrates the knowledge and ability to apply principles of mental health care.
