Position Summary

The Licensed Clinical Social Worker (LCSW) serves as a Primary Therapist within Prosperous Health's Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP). This role is responsible for managing an assigned caseload and providing comprehensive clinical care throughout the patient's treatment episode. The Primary Therapist conducts clinical assessments, develops individualized treatment plans, facilitates individual, group, and family therapy, and coordinates care with the multidisciplinary treatment team.

This position works closely with all clinical departments to ensure patients receive appropriate, evidence-based services that meet their individualized treatment needs. The LCSW plays a key role in program development, clinical collaboration, quality improvement initiatives, and ensuring that treatment services remain compliant with Level of Care (LOC) criteria, medical necessity standards, and insurance authorization requirements. The Primary Therapist is responsible for overseeing each patient's clinical progress, coordinating care transitions, and ensuring all treatment recommendations align with regulatory, accreditation, and payer guidelines.

Pay Range: $80,000–$90,000 annually (DOE)

Essential Duties and Responsibilities

• Serve as the Primary Therapist for an assigned caseload within PHP and IOP programs.
• Conduct comprehensive biopsychosocial assessments and clinical evaluations.
• Develop, implement, and regularly update individualized treatment plans based on clinical presentation and ongoing patient progress.
• Provide individual, group, and family therapy utilizing evidence-based therapeutic interventions.
• Facilitate therapeutic and psychoeducational groups in accordance with program curriculum and clinical best practices.
• Oversee each patient's clinical treatment throughout the continuum of care, ensuring all therapeutic interventions are appropriate and clinically indicated.
• Collaborate closely with psychiatrists, nursing staff, case managers, medical providers, utilization review, and other members of the multidisciplinary treatment team to coordinate comprehensive patient care.
• Participate in program development, implementation of clinical initiatives, and continuous quality improvement efforts to enhance patient outcomes and program effectiveness.
• Ensure treatment recommendations and services meet established Level of Care (LOC) criteria, medical necessity standards, and insurance authorization requirements.
• Work collaboratively with utilization review staff to support clinical documentation necessary for insurance reviews, continued stay authorizations, and payer compliance.
• Provide case management services and coordinate referrals with community providers and external resources.
• Conduct crisis intervention, suicide risk assessments, safety planning, and de-escalation as clinically indicated.
• Monitor patient progress, adjust treatment interventions as needed, and communicate clinical recommendations to the treatment team.
• Assist with discharge planning, aftercare coordination, relapse prevention planning, and continuity of care.
• Maintain accurate, timely, and compliant clinical documentation within the electronic health record (EHR).
• Advocate for patients and families while promoting engagement, treatment adherence, and recovery-oriented care.
• Ensure compliance with HIPAA, Joint Commission standards, DHCS regulations, ethical practice standards, and all applicable federal, state, and payer requirements.
• Participate in clinical team meetings, case consultations, supervision, and ongoing professional development activities.

Qualifications

• Active California Licensed Clinical Social Worker (LCSW) license required.
• Master's degree in Social Work (MSW) from an accredited institution required.
• Minimum of 2 years of clinical experience in behavioral health, substance use disorder treatment, or outpatient mental health preferred.
• Experience serving as a Primary Therapist in PHP, IOP, residential, or inpatient behavioral health settings strongly preferred.
• Thorough understanding of ASAM criteria, medical necessity, and behavioral health Levels of Care (LOC).
• Experience with insurance documentation, utilization review collaboration, and payer authorization processes preferred.
• Strong clinical assessment, treatment planning, crisis intervention, and psychotherapy skills.
• Excellent communication, organizational, and documentation abilities.
• Ability to work effectively within a multidisciplinary treatment team.
• Experience with electronic health record (EHR) systems preferred.
• Knowledge of community resources, care coordination, and discharge planning.
