Residential Services Job Description

Department: Residential Clinical Services

Reports to: Executive Director of Clinical or designated clinical supervisor

Location: Vassar residential campus

Employment status: Full-time

FLSA classification: To be determined by Human Resources

Position Summary

The Clinical Case Manager serves as both the assigned therapist and case manager for youth in Wolverine Human Services residential programs. The position is responsible for each assigned youth’s 360-degree care, integrating individual and group therapy, clinical assessment, treatment and service planning, family engagement, daily care coordination, and discharge planning. The Clinical Case Manager connects youth, families, residential staff, schools, placing agencies, and community providers to ensure that treatment goals guide care throughout placement.

The Clinical Case Manager provides individual and group therapy and coordinates the full continuum of care for an assigned caseload. Clinical services must remain within the employee’s professional license, authorized scope of practice, demonstrated competencies, and required supervision.

Intake and Individualized Planning

• Coordinate admission and orientation activities; obtain referral records, court and placement information, consent documents, educational records, and relevant medical and clinical history.

• Complete clinical and psychosocial assessments within professional scope; engage youth and families in identifying strengths, treatment needs, trauma history, cultural considerations, natural supports, and goals.

• Develop and update individualized treatment and service plans with the youth, family, and team; establish measurable goals, interventions, responsibilities, and review dates.

• Identify barriers to services and coordinate accommodations, interpretation, and other supports needed for meaningful participation.

Therapy and Integrated Care Coordination

• Serve as the primary case coordination contact for assigned youth and maintain timely communication with parents or guardians, placing agencies, caseworkers, court representatives, and other authorized partners.

• Lead treatment team meetings and case conferences; translate clinical goals into daily care strategies, coach residential staff on approved interventions, and track decisions and follow through.

• Provide individual therapy and facilitate therapeutic groups at the frequency required by each youth’s treatment plan and applicable program requirements; document participation, interventions, response, and progress.

• Provide family therapy within professional scope when indicated; engage caregivers in treatment and coordinate psychiatric care, medical services, education, recreation, mentoring, and other planned supports.

• Coordinate school communication with the Director of Client Services and educational partners; support school attendance, educational planning, and continuity during transitions.

• Promote meaningful family involvement, visits, and ongoing connection consistent with the youth’s plan, authorized contact, and applicable court orders.

• Coordinate passes and off grounds activities through established approval processes; ensure required notifications, documentation, safety planning, and follow up are completed on time.

Safety Documentation and Accountability

• Recognize changes in youth functioning and promptly communicate safety concerns, escalating needs, and service gaps to the appropriate supervisor and treatment team.

• Develop and update individualized safety and crisis plans; provide clinical intervention within scope, support staff implementation, and complete post incident clinical follow up.

• Immediately report suspected abuse, neglect, exploitation, and other reportable concerns through required external and internal reporting channels. Internal notification does not replace an individual reporting obligation.

• Maintain clinical assessments, therapy notes, treatment and service plans, progress reviews, contact notes, meeting records, and discharge documents; evaluate treatment response and revise interventions as needed.

• Complete required placing agency forms and electronic case system entries, including MiSACWIS documentation when assigned and authorized; track deadlines and address missing information before it delays care.

• Protect confidential information and share records only through approved channels with authorized individuals.

• Follow applicable licensing requirements, placement contracts, agency policies, and documentation standards; participate in chart reviews, corrective actions, and quality improvement.

Discharge and Transition Planning

• Begin discharge planning at admission and regularly review readiness, permanency goals, placement options, and barriers with the youth, family, placing agency, and treatment team.

• Coordinate referrals, appointments, records transfer, medication coordination with medical staff, school transitions, and connections to community and natural supports.

• Prepare discharge summaries and transition plans that identify continuing needs, safety supports, provider contacts, and follow up responsibilities; complete assigned post discharge contacts.
