Position Summary
The Fernandeño Tataviam Band of Mission Indians (FTBMI) is seeking a California-licensed behavioral health professional (LCSW, LMFT, or LPCC) to join the FTBMI Health & Wellness Initiative and provide outpatient behavioral health services to Tribal citizens and eligible community members.
The Licensed Behavioral Health Therapist will provide behavioral health assessments, psychotherapy, treatment planning, behavioral health education, risk assessment and safety planning, care coordination, and referrals to psychiatric and other specialty services when clinically appropriate.
This position will operate under a hybrid, telehealth-forward model, allowing the majority of appropriate behavioral health services to be delivered through telehealth while maintaining scheduled in-person availability at the FTBMI San Fernando location.
As an early clinical provider within FTBMI's developing behavioral health program, the Therapist will also collaborate with FTBMI leadership and program staff to help establish effective clinical workflows, referral pathways, documentation practices, quality measures, and culturally responsive behavioral health programming.
Essential Duties & Responsibilities
Clinical Behavioral Health Services
· Conduct behavioral health intake assessments and clinical evaluations within professional scope.
· Provide individual psychotherapy to adults and, based on training and competency, children and adolescents.
· Provide couples and family therapy when appropriate and within professional competency.
· Develop individualized treatment plans and measurable treatment goals.
· Provide ongoing psychotherapy and periodically reassess patient progress and treatment needs.
· Provide treatment for appropriate outpatient behavioral health concerns, including anxiety, depression, stress, grief and loss, trauma-related symptoms, adjustment difficulties, relationship concerns, and family-related issues.
· Utilize evidence-informed, trauma-informed, and culturally responsive therapeutic approaches.
· Administer appropriate behavioral health screening and outcome measurement tools.
· Maintain timely and accurate clinical documentation in the designated electronic health record.
· Telehealth & In-Person Services
· Provide behavioral health services through secure telehealth technology when clinically appropriate.
· Maintain scheduled in-person availability at the FTBMI San Fernando location.
· Assess the appropriateness of telehealth based on individual patient needs and clinical circumstances.
· Follow applicable telehealth consent, privacy, documentation, and patient-safety requirements.
· Facilitate in-person evaluation or referral to another level of care when telehealth or routine outpatient treatment is not clinically appropriate.
· Support an accessible service model that incorporates both telehealth and in-person behavioral health care.
· Risk Assessment & Care Coordination
· Conduct suicide, self-harm, safety, and other behavioral health risk assessments as clinically appropriate.
· Develop safety plans and follow established escalation and emergency procedures.
· Identify patients whose clinical needs exceed the scope of routine outpatient psychotherapy.
· Coordinate referrals to psychiatry, psychiatric nurse practitioners, primary care, substance-use treatment, specialty behavioral health services, crisis resources, and higher levels of care.
· Collaborate with FTBMI wellness and care-navigation staff to promote continuity of care and patient follow-up.
· Assist in developing and maintaining relationships with external behavioral health and healthcare resources.
· Community Health & Wellness
· Participate in behavioral health education, prevention, and wellness programming.
· Support community workshops, groups, and educational activities as appropriate.
· Collaborate with FTBMI program staff to identify behavioral health needs, service gaps, and barriers to accessing care.
· Support culturally responsive behavioral health programming informed by Tribal priorities and community feedback.
· Participate in interdisciplinary meetings and behavioral health program-development activities.
· Program Development, Quality & Compliance
· Assist in developing and refining clinical workflows and behavioral health servicedelivery processes.
· Provide clinical input regarding referral pathways, patient access, and continuity of care.
· Support development and implementation of clinical documentation standards and workflows.
· Participate in quality-improvement and patient-outcome measurement activities.
· Support collection of aggregate utilization and outcome information for program evaluation and applicable grant reporting.
· Maintain patient confidentiality and comply with applicable privacy requirements.
· Complete assessments, treatment plans, progress notes, discharge documentation, and other required clinical records.
· Maintain active professional licensure and required continuing education.
· Participate in provider credentialing and recredentialing activities.
· Follow applicable professional, ethical, organizational, payer, and regulatory requirements.
Minimum Qualifications
· Master's degree in Social Work, Marriage and Family Therapy, Counseling, Psychology, or another qualifying behavioral health discipline.
· Current, unrestricted California license as a:
· Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC)
· License must be active and in good standing.
· Experience providing outpatient psychotherapy and behavioral health assessments.
· Experience conducting clinical risk and safety assessments.
· Ability and willingness to provide behavioral health services through telehealth.
· Strong clinical documentation, organizational, and communication skills.
· Ability to appropriately handle confidential and sensitive health information.
· Ability to work effectively with individuals and families from diverse cultural and socioeconomic backgrounds.
· This position requires independent California licensure. Associate-level registrations (ASW, AMFT, and APCC) will not be considered for this position.
· Preferred Qualifications
· Three or more years of post-licensure behavioral health experience.
· Experience in outpatient or community behavioral health.
· Experience providing telehealth psychotherapy.
· Experience working with Native American/Tribal communities or other culturally diverse populations.
· Experience with trauma-informed and culturally responsive care.
· Experience with Medi-Cal, managed care, commercial insurance, and/or payer documentation requirements.
· Experience providing behavioral health services to adults, children, adolescents, and families.
· Experience utilizing standardized behavioral health screening and outcome measures.
· Experience coordinating psychiatric and other specialty behavioral health referrals.
· Experience participating in the development or expansion of a behavioral health program.
· Bilingual English/Spanish preferred but not required.
Working Conditions
This is a part-time Contractor role based in San Fernando, California. The position is designed to support significant use of telehealth for clinically appropriate services while maintaining scheduled in-person availability to support patients and program needs.
In-person meetings, trainings, community programs, and events may be required. Occasional evening or weekend participation may be necessary based on program and community needs.
