POSITION SUMMARY:

The Lead Social Worker provides clinical leadership, operational oversight, and advanced social work services within the Case Management Department. This role supports high-quality, patient-centered care through psychosocial assessment, care coordination, discharge planning, resource management, and interdisciplinary collaboration. The Lead Social Worker serves as the first-line clinical lead for social work staff, providing mentoring, education, consultation, and performance support while ensuring compliance with regulatory, ethical, and professional standards.  

EDUCATION / TRAINING / EXPERIENCE:

Minimum:

Master’s degree in Social Work from accredited program

Three years experience in a medical setting

Experience in case management and discharge planning.

Desired:

Experience in acute medical care, Case Management Social Work  

Licensed Clinical Social Worker (LCSW) in California.

Previous supervisory, lead, or program coordination experience.

LICENSES / CERTIFICATIONS:

Minimum:

     Current CPR Certification

SKILLS / KNOWLEDGE / ABILITIES:

Advanced skills in Clinical Social Work, including Case Management assessment, planning, intervention, and evaluation. Addresses psychosocial/disease management/resource utilization aspects of patient care including issues impacting compliance/self care related to disease or illness and cultural issues.  Strong crisis intervention skills. Knowledge of Bio-Ethics, CISM, and other committees and policies pertinent to Social Work in the medical setting.  Critical thinking and analytical skills.  Must have excellent communication and interpersonal skills to establish a high degree of rapport and professional interaction.  Must be able to maintain strict confidentiality at all times. Ability to build positive working relationships with physicians and external agencies.  Ability to communicate with multiple levels of the organization (e.g., managers, physicians, clinical, and support staff).  Knowledge of state and federal funding sources and local community services. Understanding of third-party reimbursement methodologies. Understanding of Centers for Medicare and Medicaid (CMS) Conditions of Participation (CoP) relating to the scope of Case Management and other Federal/State mandates for discharge planning. Working knowledge of regulatory and managed care requirements, hospital information systems and mechanism of utilization review.  Capable of using Microsoft Office Suite, EPIC, MIDAS, Chart Maxx and other computer programs for a variety of support functions. 

Must have the ability to utilize critical thinking skills and make independent decisions.  Must have the ability to lead, motivate, delegate, and problem solve.  Assessment, planning, intervention, and evaluation of patient care.  Demonstrates clinical SW knowledge as evidenced by completion of ACAT. Updates knowledge by completion of the Skills Fair on an annual basis, and independent study. Works autonomously to carry out the priorities of the department and the organization including high census, throughput, and interdisciplinary team facilitation. Possesses flexibility and adapts quickly to assess day to day barriers and challenges to throughput and department operations. Collaborates with LEAD RNCC multiple times a day to plan and coordinate scheduling, barrier removal, staffing, and other challenges to throughput and high quality care delivery. Organizational and multi-tasking skills are essential.   Must be able to fulfill the essential functions of the position.
