Lead Case Manager FSP-ICM
Under the direction of the Program Manager, the Lead Case Manager for the FSP-ICM program provides frontline operational support and field guidance to case managers, peer support specialists, housing staff, and rehabilitation-focused team members. The Lead Case Manager supports all assigned staff with key activities such outreach, engagement, rehabilitation services, resource coordination, housing linkage, and field-based service coordination to ensure continuity of care and effective community-based support for clients. The role supports staff and clients to ensure FSP fidelity through persistent outreach, field engagement, whole-person rehabilitation support, and flexible community-based service delivery. The Case Management Team Lead functions as a field and service coordination support role and does not provide formal administrative supervision unless specifically delegated by leadership.
POSITION REQUIREMENTS:
- Bachelor’s degree in Social Services or related field and two (2) year of experience working with adults with mental health, homelessness history and/or substance use issues; or Associates degree in Social Services and three (3) years of similar experience; or High School Diploma or. G.E.D. equivalent and three (4) years of direct services in Mental Health or community services.
- Certificate of completion for the Medi-Cal Peer Support Specialist training by an approved California Mental Health Services Authority vendor or willingness to complete within 6 months of employment.
- Must have at least two years’ experience serving participants in the full service partnership or intensive case management services program.
- One year of management/supervisory experience preferred, but not required.
- Must be able to pass California State Department of Justice criminal background investigation, Live Scan and/or fingerprinting.
- Must have a valid California Driver's license and the availability of a car with full coverage insurance.

- Ability to provide leadership and guidance to other Case Manager, Peer Support Specialist, Employment Specialist and/or Housing Specialists.
- Ability to respond quickly and effectively to client/family crises and life situations.
Primary Duties and Responsibilities:
- Conduct weekly individual meetings with assigned staff to review cases and documentation standards with assigned staff, provide guidance and support for client care and ensure staff’s adherence to best practices and agency policies and documents all meetings.
- Assist case managers and peers with ensuring outreach and engagement activities are conducted according to PSE guidelines and provide role modeling of activities when necessary.
- Assist assigned staff in the field with difficult-to-engage clients and challenging situations by role modeling, teaching skills in real time and providing support to staff who have challenges.
- Assist staff in coordinating housing linkage, transportation, resource navigation, and other types of linkage when needed.
- Assist staff by ensuring appropriate continuity of care for clients transitioning from hospitals, shelters, incarceration, and institutional settings.
- Participate weekly on assigned screening days for client walk in/call in potential client screenings and conduct field based screenings for potential FSP ICM clients to determine eligibility.
- Conduct field-based mental health rehabilitation, peer support services and targeted case management services alongside assigned staff who are in need of further training and support to engage in these activities effectively.
- Conduct training sessions during the onboarding and orientation for case management and peer staff when necessary, as well as conduct ongoing training and education to staff within the first 6 months of assigned staff’s employment or when needed to support team.
- Assist in maintaining and enhancing community resource guide and linkage information in agency’s resource manual.
- Coordinate with shelters, food resources, housing providers, and community agencies regarding client support needs.
- Ensure that assigned staff are skilled to implement recovery-oriented, strengths-based, and whole-person rehabilitation services.
- Maintain 12 hours of billing per week and meet all agency standards for documentation by providing services to assigned cases and/or providing outreach and/or direct services to clients enrolled in FSP-ICM.
- Ensure all assigned staff are meeting program billing expectations and meeting all agency standards for documentation through monitoring, training and direct support when needed.
- Attend weekly FSP ICM meetings, regularly scheduled agency wide meetings, community meetings and other meetings as assigned.
- Work collaboratively with the Department of Mental Health, Department of Probation, and other interagency and intra-agency staff to provide consistent services.
- Participate in weekly supervision with Program Manager.
- Participate and/or support staff with client crisis and urgent care issues on a 24/7 basis as required by agency when needed. Provide role modeling, support and training to staff for crisis intervention when needed or assigned.
- Participate in the On-Call schedule as scheduled by Directors.
- Additional duties and special projects may be assigned as necessary.
