Intensive Intervention Coordinator

Position Overview
The Intensive Intervention Services (IIS) Coordinator provides short-term, community-based mental health support and crisis stabilization services to adults experiencing acute mental health and/or substance use challenges. This role focuses on assessment, service planning, care coordination, crisis intervention, and connecting individuals to community resources to promote recovery and stability.

Key Responsibilities
Conduct comprehensive assessments and develop individualized service plans.
Provide community-based support, crisis intervention, and brief therapeutic interventions.
Coordinate and connect clients with medical, behavioral health, social service, and community resources.
Maintain regular contact with clients, families, and collateral providers.
Complete clinical documentation and maintain accurate records in compliance with agency and regulatory standards.
Participate in clinical supervision, team meetings, and required trainings.
Assist with crisis coverage as assigned.
Transport clients when necessary using agency vehicles or personal vehicle as approved.

Preferred Qualifications
Master's degree in Social Work, Counseling, Psychology, or related field.
Two years of experience in mental health, substance use, or human services.

Minimum Requirements
Bachelor's degree in Social Work, Counseling, Psychology, or related field.
Four years of relevant human services experience.
Strong computer and documentation skills.
Valid driver's license and ability to travel throughout the community.
Ability to obtain required background clearances.
