What You'll Do 

The Intensive Case Manager is responsible for providing comprehensive mental health assessment and case management services to formerly incarcerated clients of IMPACT program, in Queens, NY, who may have substance abuse barriers to employment by applying clinical judgement to determine appropriate treatment needs and levels of care.

Your responsibilities will include: 

Case Management 

  • Conducts intakes, screenings, and assessments.

  • Manages a caseload of program participants with mental health barriers.

  • Enter detailed case notes within 24 hours on a daily basis for all client contact / updates.

  • Works cooperatively with other staff as a member of the interdisciplinary team.

  • Provides individualized services to 100% of Participants on caseload from point of intake to exit.

  • Functions as a liaison/advocate to clients’ families and/or other agencies as needed.

  • Demonstrates appropriate treatment planning and clinical intervention for 100% of participants on caseload.

  • Conduct thorough screenings, assessments, and through a motivational interviewing process develop individualized person-centered comprehensive service plans for 100% of caseload.  

  • Determines the appropriateness of the level of care being currently provided by the treatment program for clients already in treatment, including relapse prevention.

  • Reviews the treatment history utilizing available resources.

  • Identifies available vacancies in appropriate treatment programs and makes appointments for initial interviews for clients, minimizing disruption to clients and family, where clinically appropriate.

  • Participates in case conferences with other community-based service providers, including treating physicians, clinicians, Case Management staff, housing specialists, benefits, or entitlements specialists and/or other needed supports in the community.

  • Provides crisis intervention/management, generating referrals to Mental Health Services and other provider resources, as indicated.

Facilitation

  • Orients clients about program goals, objectives, rules and regulations, client’s obligations, client’s rights, and program operations.

  • Facilitate individual and group sessions, including CBT and other clinical interventions.

  • Facilitate weekly groups and seminars related to mental health, substance use, and reentry on Rikers Island. 

Performance Management & Reporting

  • Ensures the maintenance of accurate, complete, timely and high-quality client records and reporting of client information that comply with external regulatory standards and agency policy and procedure daily.

  • Maintains exceptional recordkeeping and documents all communication and contact with at least 90% of caseload participants and external partners in FedcapCARES daily.

You're a great fit for this role if you have:

 

  • Bachelor of Social Work (BSW) minimum, Master's degree in Social Work (MSW) preferred – with consideration given to similar degrees such as Mental Health Counseling, Psychology, Vocational Rehabilitation, or Occupational Rehabilitation.

  • Strong writing skills; competencies in writing goal-based/person-centered treatment plans and progress notes as evidenced by successful completion of writing sample.

  • Bi-lingual (English/Spanish) preferred.

 

Compensation

$23.04-$28.85/hr.
