SUMMARY

The Intake Coordinator is part of a multidisciplinary clinical team, conducting phone and face-to-face screenings and intakes on-site, off-site, and via telehealth for veterans, guardsman, active-duty, and their families. The Intake Coordinator will work with the team for case assignments, consultations, and care coordination.  

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Conducts initial intake assessments with clients. 
  • Provides support to clients with completing initial consent forms and releases as required 
  • Provides warm hand-offs for clients needing external referrals
  • Coordinates case assignments with multi-disciplinary team  
  • Thoroughly and efficiently completes documentation within required timeline of 48 hours of intervention/communication
  • Collaborates with clinical team and participates in multidisciplinary team meetings
  • Provides crisis management support to clients and the team as needed
  • Provides services in-person and via telehealth platform, as required
  • Performs other duties as required

QUALIFICATIONS

  • Master’s degree in social work, Sociology, Psychology or related field is required; Oklahoma Licensure or Candidate for Licensure for one of the following is preferred:
    • Licensed Professional Counselor (LPC)
    • Licensed Marriage and Family Therapist (LMFT)
    • Licensed Clinical Social Worker (LCSW)
    • Licensed Alcohol and Drug Counselor (LADC/MH)
  • At least one year of experience providing intake coordination and/or case management services; experience working with a military or veteran population is preferred
  • Knowledge of military connected and veterans service organizations and resources
  • Ability to think analytically and evaluate the impact of case assignments
  • Excellent written and oral communication skills
  • Highly organized, detail oriented, and reliable
  • Works effectively in and contributes to a healthy team environment
  • Proficient in use of technology to perform duties
  • Displays appropriate understanding of and adherence to policies and procedures and organizational hierarchy
  • Ability to set priorities and evaluate and create solutions to work related problems
  • Must have a valid driver’s licenses and reliable transportation

PHYSICAL REQUIREMENTS

  • Must have the ability to stand or sit for long periods of time
  • Must have the ability to lift, push, or pull a minimum of 25 pounds
  • Ability to travel approximately 750% of the time
  • Ability to use telephone, PC, fax machine, copy machine, and printer
