GENERAL SUMMARY:

Comprehensive Community Services (CCS) is a voluntary program that provides support and services to youth and adults who are coping with mental health or substance abuse diagnosis. CCS is a community-based program in which the majority of services are provided in clients’ homes and communities. The program is person-centered and uses client-directed service plans to describe the individualized services that will support the consumer in achieving their recovery goals. Services are provided by teams of professionals, peer specialists, natural supports, which are coordinated by a CCS Care Coordinator.

The Intake Care Coordinator is responsible for ensuring all required initial paperwork for any new referral is submitted within a 30-day window, according to WI DHS Chapter 36.

ESSENTIAL FUNCTIONS:

• Gathers information to ensure eligibility as well as communicate program components to the client and families by coordinating in person meetings to complete the initial application, service agreement, and completing any other required paperwork.  Works closely and cooperatively with any external parties involved. 
• Establishes and maintains current computerized and manual systems to track the intake and referral of families.
• Consults with supervisory staff/MHP to determine case assignments and works together to match appropriate Care Coordinator to the consumer/client.
• Coordinates or recommend in-home services, community resources, & informal supports.
• Submits appropriate documentation to all parties involved in a timely manner/within the required time frames.
• Attends weekly staffing, trainings, meetings and clinical consultations as required.
• Maintains a caseload of up to 6 families and manages intakes as assigned.
• Available during program hours to assist families with crisis intervention. 
• Attends and completes Functional Screenings courses as required.
• Supports the goals of the Agency Mission / Vision.
• Reviews treatment plans annually, as required.
• Other duties as assigned.

MINIMUM SKILLS, KNOWLEDGE, AND ABILITIES REQUIRED:

• Bachelor’s Degree in Social Work or related Human Service field required. Master’s Degree in Social Work or related Human Service field required strongly preferred.
• At least 1 year of Social Services experience required.
• Recent CCS experience preferred.
• Fluent in another language besides English preferable.
• Proficient verbal communication and formal report writing.
• Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
• Ability to familiarize self with local community resources.
• Knowledge of DHS36 policies and procedures
• Knowledge of methods of evaluation, treatment and ability to interpret written results.
• Ability to understand and apply agency approved crisis management techniques.
• Attention to detail and good organization skills.
• WI Drivers License and valid insurance required.

WORKING CONDITIONS:

• General office environment/multiple locations to include community sites and client’s homes.
• Travel approximately 70%, including transportation of youth, using Agency’s vehicle(s).
