CTS Inc. is hiring a Qualified Professional (QP) to manage a caseload of children and youth up to age 21 in Family Foster Care and Therapeutic Foster Care (TFC) Level I and II. You will supervise, train, and support foster parents and coordinate care using the System of Care approach. You will also keep services compliant with NC Medicaid requirements. The role may combine case coordination and licensing social worker duties.
What you'll do
Case management and care coordination
Ensure each child has current assessments and is placed at the appropriate level of care.
Lead and attend Child and Family Team (CFT) meetings.
Develop and update Person-Centered Plans (PCPs) at least annually.
Coordinate services with families, DSS, DJJ, schools, and health care providers.
Submit Service Authorization Requests (SARs) and maintain authorizations with no gaps in service.
Monitor each child's mental and physical health and escalate concerns.
Protect client rights.
Foster parent supervision, training, and licensing
Provide weekly clinical and behavioral supervision in each foster home (60 minutes per child, by face-to-face and phone contact).
Train, credential, and support therapeutic foster parents.
Assist with Level II licensing.
Provide therapeutic and behavioral support to children and families who need a therapeutic home setting.
Documentation and compliance
Review and co-sign service notes, incident reports, and MARs within 24 business hours.
Enter home supervision notes within 24 business hours of each visit.
Contact all stakeholders within 48 hours of case assignment.
Complete a full record review within 7 days and a record audit monthly.
Prepare relocation, renewal, discharge, and outcome summaries.
Participate in State and MCO audits.
Crisis response and teamwork
Provide 24-hour on-call crisis support for your caseload, on a scheduled and as needed basis.
Attend staff meetings, clinical staffings, in-service training, and monthly supervision.
Represent CTS Inc. professionally with schools, courts, DSS, and community partners.
Schedule
Full-time, 40-hour work week.
Hybrid schedule: 2 days in the office (9 a.m.–5 p.m.) and 3 remote days.
Remote days include community-based services and at least 2–3 in-home visits per month per foster home, depending on the program.
Daily hours follow client needs and may include evenings, weekends, and on-call coverage.
Qualifications: Required qualifications
Bachelor's degree in a human services field (e.g., social work, psychology, counseling).
3+ years of experience with the population served.
1 year of residential experience, or demonstrated sound clinical knowledge of the service.
Qualified Professional (QP) status under North Carolina rules.
Experience with HIPAA and System of Care.
Valid North Carolina driver's license, reliable vehicle, and required auto insurance.
Strong written and verbal communication; at least 18 years of age.
Ability to participate in client activities (e.g., hiking, sports), with or without reasonable accommodation.
Employment is contingent on a criminal record check, fingerprinting, and an NC Health Care Personnel Registry (HCPR) check with no substantiated findings of abuse or neglect.
Training and certifications (obtain after hire if not current)
NCI / alternatives to restrictive interventions.
MAPP / GPS-MAPP.
CPR/First Aid, medication administration, and communicable disease prevention.
