Human Services Case Manager 2 (Disability Advocacy/Claims Processor Case Manager)

Location:  St. Petersburg Human Services Office - 647 1st Ave North St. Petersburg, FL 33701

Schedule:  Monday - Friday; 8:00 a.m. -  5:00 p.m.  Some Remote work.

This is advanced level professional office/field social work. Work includes obtaining health, financial, and social services for clients; provides assessments, develops, and implements case management plans. Employees in this class exercise considerable independent judgment in conducting comprehensive client assessments, determining needs, evaluating client strengths and weaknesses, and developing and implementing case management plans to effectively meet individual client needs to promote self-sufficiency. 

 

Incumbents will coordinate special social work programs or perform highly independent field or office work at a comparable level. Work is performed within established rules and policies and includes a high degree of public contact. Incumbent may work outside normal office hours to assure availability of services to special client population. The position reports to a senior department manager or designee.

 

What Will You Do?

 

  • Facilitates eligibility for the Pinellas County Disability Advocacy Program to assist eligible residents with filing disability claims with the Social Security Administration (SSA) to secure Social Security Disability Insurance (SSDI) and/or Social Security Insurance (SSI). 
  • Conducts comprehensive client assessments to collect functional, environmental, psycho-social, financial, employment, housing, educational, and health information as appropriate, to develop comprehensive services plans and aid the development of the social security claim.
  • Conducts research and reviews medical records to determine if clients meet SSA Listings (e.g., medical criteria used to evaluate disability claims).
  • Provides comprehensive case management involving a collaborative process of assessment, planning, implementation, and ongoing monitoring to assist clients with their unique needs to achieve their identified goals and benchmarks.
  • Provides resource and service coordination to clients.
  • Collaborates with community partners to coordinate resources and services, participate in multi-disciplinary staffings, and document coordination. 
  • Conducts fields visits to clients’ homes, appointments, court hearings, etc.
  • Acts as resource and liaison with partners, addressing questions and providing updated information about programs’ policies and procedures.
  • Manages special projects as assigned by the Section Manager.
  • Conducts training and/or presentations to community partners on the Disability Advocacy Program.
  • Accomplishes tasks and makes decisions independently with minimal supervision.
  • Answers information calls from the public and advises and refers callers in regard to a wide array of community assistance programs.
  • Checks benefits from other sources. 
  • Performs data entry and record keeping.
  • Attends meetings as needed.
  • Performs other related job duties as assigned.

What Do You Need?

Experience: Technical and professional experience in case management or a related human service field. 

Degree: Social work, psychology, sociology, or a related field.

  • 6 years’ experience as described above, OR;

  • Associate’s degree and 4 years’ experience as described above, OR;

  • Bachelor’s degree and 2 years’ experience as described above, OR;

  • Master’s or higher-level degree that includes some social casework experience or a related human service field, OR;.

  • An equivalent combination of education, training, and/or experience.

  • Possession and maintenance of a valid State of Florida Driver’s License.

  • Assignment to work a variety of work schedules including compulsory work periods in special, emergency, and/or disaster situations.

 

Highly Desirable

  • Experience processing Social Security Claims

  • Bilingual with demonstrated full fluency in written and spoken Spanish.
