Job Summary: The Housing Solutions Case Manager provides comprehensive services to residents in the Mercy House Permanent Supportive Housing Program. Responsibilities include needs assessment, individualized housing plans, referrals, advocacy, and ongoing support. The role also supports residential programming such as life skills workshops and community-building activities. This position works to promote resident self-reliance, housing stability, and trauma-informed, client-focused care.

Essential Duties and Responsibilities:
Client Services and Case Management


Manage a caseload of 25 to 30 residents and meet regularly to develop and monitor individualized housing plans with measurable goals
Conduct ongoing assessments and address barriers to housing stability
Use harm reduction, crisis prevention, trauma-informed care, and motivational interviewing techniques
Support eviction prevention and identify risks to housing stability
Conduct apartment visits to ensure proper maintenance
Provide referrals and connect residents to community resources
Maintain accurate, up-to-date client records and documentation
Participate in on-call rotation for crisis response


Resident Services


Help residents set and achieve personal and housing goals
Coordinate and facilitate life skills, financial literacy, and job readiness workshops
Organize social, recreational, and community-building activities
Develop monthly event calendars, flyers, and quarterly newsletters
Support residents in building social and community support networks
Assist with access to services through referrals and advocacy


Outreach and Collaboration


Maintain relationships with community partners, service organizations, and neighbors
Collaborate with property management to support effective communication and resident stability
Participate in team meetings, trainings, and program development


Administration


Complete documentation and reporting accurately and on time
Support data collection and program compliance requirements
Learn and apply knowledge related to mental health and substance use services
Perform other duties as assigned

QualificationsQualifications:
Education and Experience


Bachelor’s degree in Social Work or related field preferred
Relevant experience in mental health, substance use, or co-occurring disorders preferred
Candidates without a degree may qualify with at least 2 years of direct experience working with homeless, low-income, or diverse populations


General Requirements


Valid California driver’s license, insurance, and reliable vehicle required
Strong communication skills and ability to work independently


Physical Requirements


Ability to lift up to 50 pounds
Ability to sit and work at a computer for extended periods
