About the Role: The Housing Case Manager provides field and clinic-based case management to prospective and current housing program participants impacted by substance use. This position involves comprehensive assessment of client needs, development of personalized housing plans, and ongoing case management to ensure successful housing outcomes. The Case Manager collaborates closely with community partners, landlords, and service providers to facilitate access to housing programs, financial assistance, and supportive services. By advocating for clients and navigating complex housing systems, the Case Manager helps reduce homelessness and promotes long-term housing stability. Ultimately, this role contributes to improving the quality of life for housing insecure populations through compassionate, client-centered support and resource coordination.
Minimum Qualifications:
- At least one year of experience in recovery work, case management, social services, or housing support roles.
- Knowledge of housing resources, public assistance programs, and community service networks.
- Knowledge of chemical dependency, mental health, and housing and homeless service systems.
- Strong interpersonal and communication skills to effectively engage with diverse client populations and stakeholders.
- Ability to manage multiple cases simultaneously while maintaining attention to detail and meeting deadlines.
Preferred Qualifications:
- Bachelor's or Master’s degree in social work or related discipline.
- Experience working with homeless populations or individuals with complex needs such as mental health or substance use challenges.
- Experience supporting people across all stages of change, using evidence-based models in safety-focused care, substance use treatment, and recovery support.
- Proficiency with documenting case notes in electronic systems.
- Bilingual abilities, particularly in Spanish.
Responsibilities:
- Conduct housing-focused outreach, in collaboration with Daily Planet and community providers, to individuals impacted by substance use.
- Complete housing assessments, individualized housing stabilization plans, and eligibility documentation in coordination with the Housing Navigator.
- Provide ongoing field and clinic case management to support housing retention, including budgeting support, landlord communication, and connection to mainstream benefits.
- Collaborate proactively with prescribing providers, behavioral health clinicians, peer recovery specialists, medical providers, case managers, and outside agencies to coordinate patient care.
- Advocate on behalf of clients to resolve housing-related issues such as eviction prevention, lease negotiations, and access to financial assistance.
- Maintain accurate and timely documentation of client interactions, case notes, and outcomes in compliance with organizational and funding requirements.
- Participate in multidisciplinary team meetings and contribute to program development and improvement efforts
Skills: The Case Manager utilizes strong assessment and communication skills daily to understand client needs and build trusting relationships that facilitate effective support. Organizational and time management skills are essential for coordinating multiple cases, scheduling appointments, and ensuring timely follow-up. Problem-solving and advocacy skills enable the Case Manager to navigate housing systems, resolve barriers, and negotiate with landlords or service providers on behalf of clients. Collaboration skills are critical when working with multidisciplinary teams and community partners to create comprehensive housing solutions. Additionally, proficiency in documentation and data management ensures accurate record-keeping and compliance with program requirements, supporting continuous improvement and funding accountability.
