Summary of Position:

The Housing Case Manager provides direct support, assessments, and permanent housing coordination for individuals and families living with HIV/AIDS. The position is funded via the HOPWA Program. Housing Opportunities for Persons with AIDS (HOPWA) is a U.S. federal program dedicated to the housing needs of low-income people living with HIV/AIDS (PLWHA). It is administered by the U.S. Department of Housing and Urban Development (HUD). Locally, this program is supported through a grant via the City of Charlotte to provide services to an 11-county service area (Anson, Iredell, Cabarrus, Gaston, Lincoln, Mecklenburg, Rowan, and Union in North Carolina, and Chester, Lancaster and York Counties in South Carolina).


The Housing Case Manager duties include but are not limited to: Manage client caseloads, determine federal program eligibility, process short-term rental or utility assistance, and connect clients to essential medical and community care. The Housing Case Manager will build rapport with agencies serving PLWHA throughout the 11-county service area. They will also be responsible for attending (virtually and/or in-person, depending on location and schedule) local Continuum of Care (COC) meetings throughout the service area. Works with Program Manager(s) and the Director of Community Health & Prevention, to track measurable outcomes, and report to partner agencies. 


 

Performance Requirements

  1. Ability to read, write and understand the English language 
  2. Ability to work well within a diverse team, including but not limited to communicating with community partners, managing data dissemination and coordinating project-related strategies with the team
  3. Must exhibit excellent interpersonal and communication skills, both oral and written (i.e. customer service, telephone skills, and e-mail communication), which are required for interacting with program team, program participants, organization staff and healthcare team 
  4. Follow established guidelines for assessing compliance with protocol and operational procedures.
  5. Communicate effectively with a wide range of patients, operations staff, providers and other departments in a friendly, professional manner incorporating a team-based approach to healthcare – working with the physical and emotional development of all age groups
  6. Knowledge of medical office protocols/procedures
  7. Ability to plan, prioritize and complete delegated task with attention to detail
  8. Ability to work well under pressure is essential
  9. Ability to learn and retain information regarding HOPWA and other housing program requirements
  10. Familiarity with basic computer operations, including but not limited to: MS Word- Excel-Outlook-PowerPoint
  11. Ability to work successfully with indirect supervision and autonomy
  12. Ability to work with culturally diverse people
  13. Typical work hours are from 8:00am to 5:00pm. Attendance at evening and/or weekend meetings and events may be required as necessary.
  14. Must have valid driver’s license and personal transportation to travel between office/county locations.
  15. Neat, well-groomed and professional appearance


Education: Bachelor’s degree in social work or a discipline related to the field. All degrees must be received from appropriately accredited institutions. 

Experience Required: Minimum 1-year experience working within social service setting (public/community health) and/or with priority community/patient populations.

Key Responsibilities (10-Core)

  1. Process short-term rent, mortgage, or utility applications and work with finance department to process payments to prevent homelessness.
  2. Update/maintain accurate and secure client records.
  3. Perform intake/assessments on new and established clients, enrolling in eligible programs.
  4. Maintain up-to-date progress notes in timely manner, according to dept. policy.
  5. Coordinate/facilitate referrals in order to link patients to care/resources.
  6. Work with clients to develop a comprehensive housing plan to include goal setting to achieve and maintain housing and optimal client health while advocating in the best interest of the client.
  7. Perform community outreach to promote program services, while establishing community partnerships includes but not limited to attending community events, coordinating community meetings, providing educational presentations, etc.
  8. Complete and submit required reports; actively participating in QI activities, in order to evaluate program activities to improve quality of care and program efficiencies.
  9. Participates in staff/committee/agency meetings as required 
  10. Performs other clinical or administrative functions as directed by the supervisor.
