    Are you passionate about helping individuals and families overcome barriers to housing and achieve long-term stability? As a Housing Case Manager, you'll play a vital role in supporting people experiencing homelessness and housing insecurity through a Housing First, harm reduction, and low-barrier service approach.

    In this role, you'll work closely with community partners, landlords, and local service providers to connect individuals and families with housing opportunities, essential resources, and ongoing support. You'll help create pathways to stable housing while empowering clients to achieve greater independence and well-being.

    What You'll Do

    • Provide case management services to individuals and families experiencing homelessness or housing instability.
    • Implement Housing First, harm reduction, and low-barrier service models to support client success.
    • Participate in Coordinated Entry and Case Conferencing meetings to identify housing solutions and coordinate care.
    • Assist clients with locating and securing safe, affordable housing.
    • Support households in transitioning to permanent housing and maintaining housing stability.
    • Connect clients with community resources, benefits, healthcare, employment services, and other support systems.
    • Refer and transport clients to community services and appointments as needed.
    • Build and maintain positive relationships with local landlords and housing providers.
    • Ensure compliance with housing grant requirements and program guidelines.
    • Accurately document client interactions, outcomes, and required data in a timely manner.
    • Serve as a knowledgeable resource for clients, colleagues, and community partners on housing-related services and resources.
    • Provide support to tenants residing in Low-Income Housing Tax Credit (LIHTC) properties.

    Education & Experience

    Candidates must meet one of the following qualifications:

    • Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field and experience providing case management services, preferably in housing or social services;

    Employment Requirements

    • Valid driver's license.
    • Acceptable driving record.
    • Current automobile insurance.
