This program follows the Assertive Community Treatment model and partners with mental health, substance use disorder, and medical staff to outreach to homeless individuals in the community, camps, or shelters that are living with severe behavioral health disorders. The Homeless Outreach Stabilization and Transition (HOST) Outreach Case Manager II provides support and case management services to eligible individuals who are living with serious substance use disorders or with co-occurring behavioral health conditions, are experiencing homelessness, and whose severity of behavioral health symptom acuity level creates barriers to accessing and receiving conventional behavioral health services and outreach models.
The ideal candidate for our Outreach Case Manager II is able to engage the most vulnerable individuals in a non-judgmental manner and harm reduction lens to provide support, case management, and appropriate participant driven treatment planning. The ideal candidate enjoys being outside engaging with individuals for a majority of their work day in non-traditional treatment settings and working within a multidisciplinary team.

Thurston Mason Behavioral Health Administrative Organization (TMBH-ASO) and Olympic Health and Recovery Services (OHRS) is a quasi-governmental organization that is responsible for the network of behavioral health crisis services in Thurston and Mason counties, and is a licensed behavioral health agency with multiple programs assisting the most vulnerable population in our community through crisis services, jail services, hospital diversion, and housing.
This position reports to HOST Program Supervisor.
Position Schedule: Tuesday-Friday 7:30am-6:00pm
Salary Range: $38.10 - $46.01 hourly based on education, experience, and parity within the organization.

Overview of Major Responsibilities
Outreach and engage the most vulnerable individuals to assist with services, addressing behavioral and physical health needs.
Provide support and case management to individuals to increase stability, assist to obtain housing, and transition individuals into long-term services for behavioral health conditions.
Provide mental health screening and/or evaluation as appropriate.
Develop and maintain up to date client driven treatment plan goals and continue to reassess and modify those plans as needed.
Assist the target population served with obtaining basic resources such as shelter, food, medical services, behavioral health treatment, jobs and other social and human services as needed.
Researches and coordinates availability of community resources to provide support and encouragement towards a path of recovery.
Maximizes client choice, self-determination, decision making in the planning, delivery, evaluation and treatment, rehabilitation, and support services.
Provides supported employment and housing support services, including accessing Medicaid, as appropriate.
Maintains physical security of confidential materials and assigned equipment.
Demonstrates proficiency in utilization of electronic medical record system.
Maintains timely and accurate documentation within the electronic health record.
Maintains individual service records for enrolled individuals to include completed assessment, determination of eligibility, service plan that includes goals utilizing person centered approach and progress notes.
Provide outreach, engagement and liaison support to those people that are seen on a recurring basis which may require short term intensive case management. Facilitate services for high-need individuals across all involved systems of care.
Monitor contact behaviors and progress, provide assistance toward attaining pre-determined goals, and schedule appointments.
Promote best practices in treatment approaches, support systems, and interventions.
Meet with and interview contacts, families, and other care providers to assess needs and eligibility of services. Advocate needs of contacts within and outside system; liaison between contact, caregivers, and service providers. Provide client-level and system-wide troubleshooting and advocacy.
Follow-up with identified individuals in an effort to bridge gaps between local community, providers, law enforcement.
Communicates effectively with co-workers in dynamic environment.
Demonstrates the OHRS mission, purpose, values and beliefs in everyday language and contact with the internal and external stakeholders.
Performs other duties as assigned.
QualificationsEducation and Experience
Meets WA state qualifications to be a Mental Health Professional.
Master’s degree or further advanced degree in counseling or one of the social sciences from an accredited college or university. Or Bachelor’s degree in counseling in one of the social sciences and five (5) years of direct behavioral health experience.
WA Department of Health license or associate license as a Social Worker, Mental Health Counselor, Marriage and Family Therapist, Psychologist or BSN/RN. Or WA DOH Licensed or Certified Agency Affiliated Counselor license.
Must be able to pass WA State Patrol background check, OIG, and other background checks as required by OHRS contractual requirements.
Experience in outreach and working with individuals who are houseless is preferred.
Current Washington State Driver’s License, proof of insurance or have requested and obtained an appropriate accommodation. Satisfactory drivers record is required.
Skills and Abilities
Familiarity and knowledge of best practices for alcohol/drug abuse treatment and prevention programs, mental health and developmental disabilities programs as well as with local social service agencies.
Familiarity and knowledge of Assertive Community Treatment (ACT) model.
Ability to establish and maintain trusting and supportive relationships as well as provide inspiration and motivation to individuals transitioning towards recovery.
Strong written and verbal communication skills, as well the ability to work with people of diverse backgrounds and/or cultures.
Ability to work in a cooperative and collaborative manner as a team member with community providers, the client, and the client’s family and support network.
Ability to work with general office and computer programs. Microsoft Office and Electronic Health Records experience desired.
Ability to work in high stress, high demand environment while communicating clearly and effectively
Ability to provide clear documentation of assessments and dispositions, and meet documentation requirements in a timely way

Working Conditions
Outreach to individuals who may be living in the community, sanctioned and non-sanctioned camps, and in shelters.
Often performed under physically demanding, stressful, and environmentally diverse conditions, and requires the ability to adjust quickly to changing priorities and demands.
When in the community, does not physically interact with clients in the event of assaultive behavior, but works to maintain safety in accordance with the principles of crisis de-escalation and safety training.
Regular, in person, predictable attendance necessary to maintain care support to individuals.
Regular use of fleet vehicles and/or personal vehicle required for service in a two-county regional area.
Moderate physical exertion is required. May be required to sit, stand, squat, walk short to moderate distances on uneven terrain, lift or move boxes/equipment weighing up to 25 lbs.
Outreach is performed in outdoor settings and subject to weather conditions.
