Position Summary

The Medical Social Worker provides comprehensive psychosocial support to hospice patients and their families/caregivers. Working collaboratively with the interdisciplinary care team, this position assesses patient and family needs, coordinates community resources, provides counseling and support, and contributes to individualized care plans that promote comfort, dignity, and quality of life.

Duties & Responsibilities:

  • Conduct comprehensive psychosocial assessments to identify emotional, social, environmental, and resource-related needs of patients and families.

  • Provide compassionate counseling and support to help patients and caregivers navigate grief, stress, adjustment challenges, and end-of-life concerns.
  • Develop and implement individualized interventions that address identified psychosocial needs and support overall well-being.
  • Connect patients and families with community resources, financial assistance programs, support services, and other valuable resources.
  • Serve as an advocate and liaison between patients, caregivers, healthcare providers, and community organizations.
  • Collaborate closely with nurses, physicians, chaplains, and other interdisciplinary team members to deliver high-quality, patient-centered care.
  • Educate patients, families, and community partners regarding available services and support programs.
  • Maintain accurate and timely clinical documentation in accordance with organizational and regulatory standards.
  • Participate in interdisciplinary team meetings and contribute to the development and ongoing evaluation of individualized care plans.
  • Build and maintain relationships with community agencies and organizations to enhance available support services.
  • Support quality improvement initiatives and actively participate in performance improvement activities.
  • Perform other duties as assigned to support patient care and organizational goals.

REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

  • A graduate of a master’s program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in §418.114(b)(3).
  • Minimum of one year's experience in health care, hospice experience preferred.  
  • Understands hospice philosophy, and issues of death/dying. Experience in a hospice care preferred
  • Demonstrates good verbal and written communication, and organization skills.
  • Possesses and maintains current CPR Certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
  • Knowledge of HomeCare/HomeBase; Pointcare EMR system preferred but not mandatory.
