Solace Hospice & Palliative Care Inc is looking for a compassionate and dedicated Hospice Medical Social Worker to join our team in Saint Charles, IL. In this meaningful role, you'll provide essential emotional, social, and practical support to patients and families navigating end-of-life care. Responsibilities:
- Conduct psychosocial assessments to evaluate patient and family needs, strengths, and coping abilities
- Develop and implement individualized care plans in collaboration with the interdisciplinary hospice team
- Provide counseling and emotional support to patients and families related to grief, bereavement, and end-of-life adjustment
- Connect patients and families with community resources, financial assistance programs, and support services
- Assist with advance care planning, including advance directives and healthcare proxy documentation
- Participate in interdisciplinary team meetings to coordinate holistic, patient-centered care
- Provide bereavement follow-up support to families following the loss of a loved one
- Maintain accurate and timely clinical documentation in compliance with hospice regulations
Requirements:
- Master's degree in Social Work (MSW) from an accredited program required
- Current licensure as an LSW or LCSW in the state of Illinois
- Prior experience in hospice, palliative care, or a related healthcare setting preferred
- Strong knowledge of end-of-life care, grief counseling, and bereavement support
- Excellent communication, interpersonal, and crisis intervention skills
- Ability to work independently and collaboratively within an interdisciplinary team
- Valid driver's license and reliable transportation for home visits in the Saint Charles, IL area
