As a Social Worker, you will Plans, organizes, and performs the daily operation of the Social Services Department within Home Health and Hospice.  Ensures compliance with Blue Skies of Texas standards and applicable federal and state regulations.  Coordinates and implements social services and psycho-social needs for patients and their families.  Responsible for the interface with family including the admission, transfer and discharge process.

 

 

 

What you will do in this role: 

 


Identifies and provides the patient and caregivers psychosocial needs in accordance with the Plan of Care (POC).
Makes an initial assessment as indicated and appropriate, and ensures it is completed within five days of admission in accordance with the Medicare Conditions of Participation.
Assists the patient and caregivers with advance directives.
Assesses and strengthens the patient and caregivers’ coping skills.
Connects the patient and caregiver to community resources.
Provides interventions for specific symptom relief (such as fear, grief, anger, etc)
Screens for psychopathology and provides education and interventions accordingly
Enhances/strengthens the family system
Assesses and refers for bereavement services
Provides assistance with housing and safety issues as needed
Helps the patient and caregiver to access financial, legal, or other services as needed.
Provide holistic, family centered care across treatment settings, consulting and collaborating with the interdisciplinary team.
Assists with funeral planning as needed.
Interfaces with the Blue Skies’ Resident Counselors to address resident concerns and issues as appropriate.
Ensures the coordination of Social Services with the services of other departments.
Attends interdisciplinary team meetings and contributes to Social Services care plans.
Enables psycho-social functioning using varied treatment interventions (group, individual, family).
Informs the Administrator about legal, ethical, and patient satisfaction issues.
Utilizes infection control measures such as hand washing, standard precautions, and personal protective equipment.
Participates in facility Quality Assessment Process Improvement program.
Attends in-services and continuing education programs to maintain proficiency.
Shares on-call duties with other personnel.
Performs other duties as assigned.


 
QualificationsQualifications: 

 


Required: Masters Degree in social work or related field. Knowledge of Medicare regulations for home health and hospice preferred.
Training/Experience: Must have at least one year experience as a medical social worker within the last 24 months.
License/Certification: licensed social worker in the state of Texas. Must have a valid Texas Drivers license and current liability insurance.
Job knowledge: Awareness of the special needs of the older adult, knowledge of psycho-social and relational needs assessments with strong problem-solving skills.
