Location: South San Jose
As a Home Health MSW, you will:
Assess patients’ social, emotional, and environmental needs and identify appropriate community resources, in-home support services, and other available assistance.
Collaborate with physicians, case managers, and interdisciplinary team members to incorporate social and emotional needs into the patient’s Plan of Care (POC).
Provide counseling, education, and support to patients and families/caregivers regarding social needs, community resources, advance directives, and available services.
Connect patients and families with appropriate community resources and coordinate services to support patient well-being and independence.
Monitor and communicate changes in patients’ social and support needs to the care team and document relevant findings in the patient health record.
Participate in discharge planning and care coordination to support a safe and effective transition from home health services.
Maintain accurate, timely, and confidential electronic clinical documentation in accordance with Lorian policies, HIPAA, and applicable regulations, while participating in continuing education and in-service programs.
Required Qualifications
Graduate of an accredited school approved by the Council on Social Work Education.
Master’s degree from a school accredited or approved by the council on Social Work Education.
Valid driver’s license and insured personal vehicle (per Lorian requirements)
Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
This position is based on a pay per visit compensation model. Pay per visit ranges from $65 to $110 visit commensurate with experience and depending on type of visit.
