Home Health Care Social Worker Responsibilities & Duties

  • Provide emotional and practical support to patients and their families in their homes
  • Assess patients' needs and develop individualized care plans
  • Link patients with community resources and services
  • Collaborate with interdisciplinary healthcare teams to provide holistic care
  • Maintain accurate and up-to-date patient records
  • Stay current with advancements in home health care and social work practices

Home Health Care Social Worker Qualifications & Skills

  • Master's degree in Social Work (MSW)
  • Licensed Clinical Social Worker (LCSW) certification
  • Experience in health care or home health care settings
  • Strong understanding of health care systems and policies
  • Proficiency in electronic health records (EHR) systems
  • Fluency in multiple languages
  • Advanced training in geriatric care or chronic illness management
  • Excellent interpersonal and communication skills
  • Ability to work independently and manage a caseload
  • Bachelor's or Master's degree in Social Work
  • Licensed in good standing as a social worker
  • 2+ years of experience in home health care or related field
  • Excellent communication and interpersonal skills
  • Strong organizational and problem-solving skills
  • A commitment to patient-centered care and advocacy
  • Previous Home health experience that works with Medicare, Medicaid,  and/or VA patients
